摘要
目的:分析老年尿结石患者术后感染的肾盂尿病原菌检出情况及耐药性。方法:纳入2021年1月—2023年8月本院收治的92例尿结石术后感染患者的临床资料,分析患者肾盂尿病原菌检测情况,并实施耐药性检验,分析其耐药结果。结果:92例尿结石术后感染患者肾盂尿病原菌检测中剔除重复感染菌种,共培养出病原菌87株,其中以革兰阴性菌株为主,革兰阴性菌株中以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌为主;革兰阳性菌以粪肠球菌、屎肠球菌为主。经耐药性检验,革兰阴性菌中大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌对莫西沙星、环丙沙星、阿莫西林、哌拉西林、氨苄西林、替卡西林、复方磺胺甲唑的耐药率高。革兰阳性菌中粪肠球菌、屎肠球菌对苯唑西林、青霉素、氨苄西林、庆大霉素的耐药率较高。结论:革兰阴性菌为引发老年尿结石患者术后感染的主要病原菌,临床需监测其分布情况及耐药情况,实施合理用药。
Objective:To analyze the detection and resistance of pyeluria pathogens in elderly patients with urinary calculi.Methods:The clinical data of 92 patients admitted to our hospital from January 2021 to August 2023 were included in the detection of pyeluria pathogens,and the resistance test and drug resistance results were analyzed.Results:A total of 87 strains of pathogenic bacteria were cultured from 92 patients with urinary calculi afer operation.Among them,Gram-negative strains were the main ones,and Escherichia coli,Klebsiella pneumoniae and Pseudomonas aeruginosa were the main Gram-negative strains.Gram-positive bacteria were mainly Enterococcus faecalis and Enterococcus faecium.According to the drug resistance test,Escherichia coli,Pseudomonas aeruginosa and Klebsiella pneumoniae in Gram-negative bacteria were highly resistant to moxifloxacin,ciprofloxacin,amoxicillin,piperacillin,ampicillin,ticarcillin and compound sulfamethoxazole.The resistance rates of Enterococcus faecalis and Enterococcus faecium to oxacillin,penicillin,ampicillin and gentamicin were higher in Gram-positive bacteria.Conclusion:Gram-negative bacteria are the main pathogenic bacteria causing postoperative infection in elderly urinary calculi,monitoring their distribution,drug resistance and rational drug use.
