摘要
目的:探讨弥散加权成像(diffusion-weighted imaging,DWI)不同参数术前评估子宫内膜癌淋巴脉管间隙浸润(lymphovascular space invasion,LVSI)的价值。方法:回顾并分析146例子宫内膜癌患者的临床及病理学资料,其中LVSI(-)组109例,LVSI(+)组37例。测量两组患者表观弥散系数最大值(maximum apparent diffusion coefficient,ADC_(max))、ADC最小值(ADC_(min))、ADC平均值(ADC_(mean))这3个参数,比较两组间各参数的差异,采用受试者工作特征(receiver operating characteristic,ROC)曲线比较ADC_(min)及ADC_(mean)鉴别子宫内膜癌LVSI的诊断效能。结果:两组患者ADCmax差异无统计学意义(P>0.05),两组间ADC_(mean)(F=13.832,P<0.001)及ADC_(min)(F=19.927,P<0.001)差异有统计学意义。ADC_(min)、ADC_(mean)预测子宫内膜癌LVSI的ROC曲线的曲线下面积分别为0.853、0.772。根据最佳诊断阈值,ADC_(min)及ADC_(mean)评估子宫内膜癌LVSI的灵敏度分别为86.5%及62.2%,特异度为71.6%及82.6%。ADC_(min)评估LVSI的诊断效能明显高于ADC_(mean)(Z=2.221,P=0.026)。结论:ADC_(min)及ADC_(mean)均有助于术前评估子宫内膜癌LVSI,且ADC_(min)的诊断效能更高。
Objective:To investigate the value of diffusion-weighted imaging(DWI)with different parameters in preoperative evaluation of lymphovascular space invasion(LVSI)of endometrial carcinoma(EC).Methods:The clinical and pathological data of 146 cases of EC were retrospectively analyzed,including 109 cases of LVSI(-)and 37 cases of LVSI(+).Maximum apparent diffusion coefficient(ADCmax),ADC_(min)and ADC_(mean)of the two groups of patients were measured,and the differences of each parameter between the two groups were compared.Receiver operating characteristic(ROC)curve analysis was carried out to evaluate the diagnostic performance of ADC_(min)and ADC_(mean)in evaluating LVSI status in EC.Results:No statistically significant difference was found in ADC_(max) between the two groups(P>0.05).There was significant difference in ADC_(mean)(F=13.832,P<0.001)and ADC_(min)(F=19.927,P<0.001)between the two groups.The area under curve(AUC)of ADC_(min),ADC_(mean)for evaluating LVSI status in EC were 0.853 and 0.772 respectively.According to the optimal cut point,the sensitivity and specificity value of ADC_(min)and ADC_(mean)were 86.5%and 62.2%,71.6%and 82.6%.The diagnostic performance of LVSI evaluated by ADC_(min)was significantly higher than ADC_(mean)(Z=2.221,P=0.026).Conclusion:Both ADC_(min)and ADC_(mean)are helpful for preoperative evaluation of LVSI status in EC,and ADC_(min)has a higher diagnostic performance.
