摘要
目的 构建“针灸科嵌入式查房”模式,研究其对于提升糖尿病周围神经病变(DPN)诊治及预后的应用效果。方法 医院于2021年10月引入并实施“针灸科嵌入式查房”,开始改革针灸诊疗模式。实施前收集2019年10月至2021年10月我院内分泌科收治的DPN患者85例,实施后收集2021年10月至2023年6月我院内分泌科收治的DPN患者96例,共181例患者。对比改实施前后DPN患者医疗服务能力(平均住院日、平均住院费用、中医非药物治疗费用、中医非药物治疗率)、患者就医感受度(7 d满意度、30 d满意度)等相关质量安全指标。结果 与实施前相比,实施后平均住院日缩短,平均住院费用减少,平均中医非药物治疗费用增加,中医非药物治疗率提高(P <0.05)。实施后,内分泌科、针灸科的7 d满意度、30 d满意度均比实施前上升(P <0.05)。结论 实施“针灸科嵌入式查房”模式能够对该病种增强医疗服务能力,提升临床医疗效能,缩短住院时间,减少住院费用,提升患者就医感受。
Objective To investigate the effectiveness of the “Embedded Acupuncture Room” model in improving the diagnosis and prognosis of diabetic peripheral neuropathy(DPN).Methods In October 2021,the hospital introduced and implemented the “Embedded Checkup in Acupuncture Department” to start reforming the acupuncture treatment model.Before implementation,85 DPN patients admitted to the endocrinology department of the hospital from October 2019 to October 2021 were collected,and after implementation,96 DPN patients admitted to the endocrinology department of the hospital from October 2021 to June 2023 were collected,totaling 181 patients.We compared the medical service capacity(average hospitalization day,average hospitalization cost,cost of non-pharmacological treatment in TCM,rate of non-pharmacological treatment in TCM),patients' perception of medical treatment(7-day satisfaction,30-day satisfaction) and other related quality and safety indicators of DPN patients before and after the change of implementation.Results Compared with the pre-implementation period,the average hospitalization day was shortened,the average hospitalization cost was reduced,the average TCM non-pharmacological treatment cost was increased,and the TCM non-pharmacological treatment rate was increased after the implementation(P<0.05).After implementation,the 7-day satisfaction and 30-day satisfaction of endocrinology and acupuncture increased compared with the pre-implementation period(P<0.05).Conclusions The implementation of the “Embedded Acupuncture Room” model can enhance the medical service capacity of this disease,improve the effectiveness of clinical care,shorten the hospitalization time,reduce hospitalization costs,and improve the patient's experience of medical treatment.
