老年心肌梗死患者外周血单个核细胞中c-fos c-myc表达及临床意义

Expressions and clinical significance of c-fos and c-myc in peripheral blood mononuclear cells of elderly patients with myocardial infarction

摘要 目的分析老年心肌梗死(MI)患者外周血单个核细胞中c-fos与c-myc表达,并探讨二者与老年MI的关系。方法将78例老年MI患者设为研究1组,73例稳定型心绞痛患者设为研究2组,同期纳入74名健康体检志愿者作为对照组。采用实时荧光定量聚合酶链反应检测三组被试者外周血单个核细胞中c-fos、c-myc水平。采用Pearson法分析c-fos、c-myc与血糖、高密度脂蛋白、三酰甘油的相关性,采用Logistic回归分析探讨MI预后的影响因素。结果研究1组被试者血糖水平高于对照组,三酰甘油水平及c-fos、c-myc水平高于研究2组和对照组,高密度脂蛋白水平低于研究2组和对照组;研究2组被试者高密度脂蛋白水平低于对照组,c-fos、c-myc水平高于对照组(P<0.05)。MI患者外周血单个核细胞中c-fos、c-myc水平与血糖、三酰甘油水平均呈正相关(P<0.01),与高密度脂蛋白呈负相关(P<0.01)。预后不良组患者c-fos、c-myc水平、三酰甘油水平高于预后良好组(P<0.01)。Logistic回归分析结果显示,c-fos、c-myc是影响MI患者预后不良的危险因素(P<0.01)。结论老年MI患者外周血单个核细胞中c-fos、c-myc呈高表达,二者与MI病情发展密切相关,c-fos、c-myc对预测老年MI不良预后具有一定的作用。 Objective To analyze c-fos and c-myc expression in peripheral blood mononuclear cells of elderly patients with myocardial infarction(MI)and explore their relationships with the senile MI.Methods Seventy-eight elderly MI patients were assigned to study group 1,73 stable angina ones to study group 2,and 74 health check-up volunteers to control group during the same period.C-fos and c-myc levels in peripheral blood mononuclear cells were detected in 3 groups using quantitative realtime PCR(qRT-PCR).The correlations of c-fos and c-myc with blood glucose,high-density lipoprotein(HDL),and triglycerides(TG)were analyzed using Pearson method,influencing factors of MI prognosis done using Logistic regression analysis.Results The blood glucose level was higher in study 1 than control group,TG level and c-fos and c-myc expression were higher and HDL lower in study 1 than study 2 and control group;HDL level was lower and c-fos and c-myc expression were higher in study 2 than control group(P<0.05).C-fos and c-myc expression levels in peripheral blood mononuclear cells of MI patients were positively correlated with blood glucose and TG levels(P<0.01)and negatively with HDL(P<0.01).C-fos and c-myc expression and TG level were higher in patients with poor prognosis than ones with good prognosis(P<0.01).Logistic regression analysis showed that c-fos and c-myc were risk factors influencing poor prognosis in MI patients(P<0.01).Conclusion The expressions of c-fos and c-myc in peripheral blood mononuclear cells of elderly MI patients are high and are closely related to MI progression,c-fos and c-myc play a certain role in predicting the poor prognosis of elderly MI.
