摘要
目的探讨跨上皮快速角膜胶原交联术(CXL)治疗进展期圆锥角膜患者的效果。方法选取2017年10月至2021年3月于哈尔滨爱尔眼科医院诊治的60例(65眼)进展期圆锥角膜患者,均行跨上皮快速CXL,比较患者治疗前后裸眼视力(UCVA)、最佳矫正视力(BCVA)、屈光状态、角膜透明及眼压。结果术后3、6个月,患者UCVA均高于术前及术后1、12个月,且术后12个月高于术后1个月,差异有统计学意义(P<0.05)。术后3、6个月,患者BCVA高于术前及术后1、12个月,且术后6个月高于术后3个月,术后12个月高于术后1个月,差异有统计学意义(P<0.05)。术前及术后不同时间点的球镜度比较差异无统计学意义;术后1个月柱镜度低于术前,术后12个月柱镜度高于术后1、3、6个月,差异有统计学意义(P<0.05)。术前及术后1、3、6、12个月,患者眼压比较差异无统计学意义。随访期间,所有患者未出现角膜感染,晶状体保持透明。结论跨上皮快速CXL治疗进展期圆锥角膜效果显著,可有效改善患者视力水平,具有较高应用价值。
Objective To explore the effect of transepithelial rapid corneal collagen crosslinking(CXL)surgery in the treatment of advanced ker-atoconus.Methods 60 patients(65 eyes)with advanced keratoconus treated at Harbin Aier Ophthalmology Hospital from October 2017 to March 2021 were selected,all of whom underwent transepithelial rapid CXL.The naked eye visual acuity(UCVA),best corrected visual acuity(BCVA),re-fractive status,corneal translucency,and intraocular pressure were compared before and after treatment.Results At 3 and 6 months after surgery,UCVA was higher than before surgery and 1 and 12 months after surgery,and 12 months after surgery was higher than 1 month after surgery,the dif-ferences were statistically significant(P<0.05).At 3 and 6 months after surgery,BCVA was higher than before surgery and 1 and 12 months after surgery,and 6 months after surgery was higher than 3 months after surgery,12 months after surgery was higher than 1 month after surgery,the differ-ences were statistically significant(P<0.05).There were no significant differences in spherical scope between preoperative and postoperative time points.One month after surgery,the degree of column mirror was lower than before surgery,12 months after surgery was higher than 1,3,6 months after surgery,the difference was statistically significant(P<0.05).There was no significant difference in IOP between patients before operation and 1,3,6 and 12 months after operation.During follow-up,all patients did not develop corneal infection and the lens remained clear.Conclusion Tran-sepithelial rapid CXL can effectively improve the visual acuity of patients in the treatment of advanced keratoconus,and its application value is sig-nificant.
