IGF-1、PGRN、miR-922在急性淋巴细胞白血病患儿血清中表达及相关性分析

摘要 目的:分析胰岛素样生长因子-1(IGF-1)、颗粒蛋白前体(PGRN)、微小RNA-922(miR-922)在急性淋巴细胞白血病(Acutelymphoblasticleukemia,ALL)患儿血清中表达及相关性。方法:选取2020年4月-2023年1月救治中心和新乡医学院第三附属医院收治的110例初诊ALL患儿为观察组,根据ALL危险度分层将ALL患儿分为标危(29例)、中危(45例)、高危(36例),另选取同期110例健康体检儿童为对照组,比较两组、不同危险度分层ALL患儿血清IGF-1、PGRN、miR-922水平,分析三项血清指标与ALL患儿危险度及急性生理与慢性健康评分(APACHEⅡ)的相关性及与ALL患儿病情中、高危的关系。结果:观察组血清IGF-1、PGRN、miR-922水平均高于对照组(P<0.05);ALL患儿不同危险度分层血清IGF-1、PGRN、miR-922水平比较:标危<中危<高危(P<0.05);血清IGF-1、PGRN、miR-922水平与ALL患儿危险度及APACHEⅡ评分均呈正相关(P<0.05);危险度分析显示,血清IGF-1、PGRN、miR-922为高值时会增加患儿中、高危风险,RR值分别为4.337、3.139、4.475,95%CI分别为2.249~8.364、1.803~5.467、2.189~9.151(P<0.05)。结论:血清IGF-1、PGRN、miR-922水平与ALL发生发展密切相关,且在评估ALL危险度方面具有良好价值。 Objective:To analyze the expression and correlation of insulin-like growth factor-1(IGF-1),granule protein precursor(PGRN),and microRNA-922(miR-922)in the serum of children with acute lymphoblastic leukemia(ALL).Methods:A total of 110 newly diagnosed ALL children admitted to the treatment center and the Third Affiliated Hospital of Xinxiang Medical College from April 2020 to January 2023 were selected as the observation group.According to the risk stratification of ALL,all children were divided into standard risk(29 cases),medium risk(45 cases),and high risk(36 cases).In addition,110 healthy children who underwent physical examinations during the same period were selected as the control group.The serum IGF-1,PGRN,and miR-922 levels of the two groups and different risk stratification of ALL children were compared.The correlation between the three serum indicators and the risk of ALL children,as well as the acute physiological and chronic health score(APACHE II),and their relationship with the condition of medium and high risk ALL children were analyzed.The serum levels of IGF-1,PGRN,and miR-922 in the observation group were higher than those in the control group(P<0.05);Comparison of serum IGF-1,PGRN,and miR-922 levels in children with ALL at different risk levels:standard risk<moderate risk<high risk(P<0.05);The levels of serum IGF-1,PGRN,and miR-922 were positively correlated with the risk and APACHE II score of ALL patients(P<0.05);Risk analysis showed that high levels of serum IGF-1,PGRN,and miR-922 increased the risk of medium and high risk in children,with RR values of 4.337,3.139,and 4.475,and 95%CI values of 2.249 to 8.364,1.803 to 5.467,and 2.189 to 9.151,respectively(P<0.05).Conclusion:Serum levels of IGF-1,PGRN,and miR-922 are closely related to the occurrence and development of ALL,and have good value in evaluating the risk of ALL.
