摘要
目的调查COPD患者疾病不确定感、慢性病资源利用和心理一致感的现况,并分析心理一致感在慢性病资源利用与疾病不确定感间的中介效应。方法采用便利抽样方法,选取201例COPD患者为研究对象,采用一般资料调查表、疾病不确定感量表、慢性病资源利用问卷及心理一致感量表进行调查。采用Pearson相关分析法分析疾病不确定感、慢性病资源利用与心理一致感间的相关性,并采用结构方程模型分析心理一致感在慢性病资源利用与疾病不确定感间的中介作用。结果COPD患者疾病不确定感总分为(77.41±8.72)分、慢性病资源利用总均分为(2.97±0.03)分、心理一致感总分为(63.43±6.16)分。患者疾病不确定感与慢性病资源利用呈负相关(r=-0.744,P<0.01),与心理一致感呈负相关(r=-0.671,P<0.01);慢性病资源利用与心理一致感呈正相关(r=0.571,P<0.01)。慢性病资源利用通过心理一致感对疾病不确定感的间接效应为-0.313,占总效应的33.6%。结论COPD患者疾病不确定感处于中等水平,心理一致感在慢性病资源利用与疾病不确定感间起部分中介作用。
Objective To investigate the current status of disease uncertainty,utilization of chronic disease resources,and psychological coherence in patients with COPD,and analyze the mediating effect of psychological coherence between the utilization of chronic disease resources and disease uncertainty.Methods Convenient sampling method was used to select 201 COPD patients as the research subjects.General information questionnaire,Mishel's Uncertainty of Illness Scale,Chronic Illness Resource Survey,and Sense of Coherence Scale-13 were used for investigation.Pearson correlation analysis was used to analyze the correlation between disease uncertainty,utilization of chronic disease resources,and psychological coherence.Structural equation modeling was used to analyze the mediating effect of psychological coherence between the utilization of chronic disease resources and disease uncertainty.Results The total score of disease uncertainty in COPD patients was(77.41±8.72),the total average score of utilization for chronic disease resources was(2.97±0.03),and the total score of psychological coherence was(63.43±6.16).Disease uncertainty in COPD patients was negatively correlated with utilization of chronic disease resources(r=-0.744,P<0.01)and negatively correlated with psychological coherence(r=-0.671,P<0.01).Utilization of chronic disease resources was positively correlated with psychological coherence(r=0.571,P<0.01).The indirect effect of utilization for chronic disease resources on disease uncertainty through psychological coherence was-0.313,accounting for 33.6%of the total effect.Conclusion Disease uncertainty in COPD patients is at a moderate level,and psychological coherence partially mediates between the utilization of chronic disease resources and disease uncertainty.
