摘要
目的观察六味地黄汤结合敏使朗治疗后循环缺血性眩晕(PCIV)的效果。方法选用本院2021年1月—2023年1月接收的120例PCIV患者,通过双盲随机分组方式进行分组,分为60例行敏使朗治疗的对照组,以及60例行六味地黄汤结合敏使朗治疗的观察组,分析两组患者中医症候积分、眩晕评估评分量表(DARS)和眩晕障碍表(DHI)评分、血流动力学指标、症状消失时间、治疗总有效率、治疗满意度以及不良反应发生率。结果治疗后,观察组目眩头晕、耳聋耳鸣、乏力倦怠、头重如蒙、呕吐恶心等各项中医证候积分、DARS评分、DHI评分以及不良反应发生率均低于对照组(均P<0.05);观察组基底动脉(BA)、右椎动脉(RVA)、左椎动脉(LVA)均较对照组更高,阻力系数(RI)更低(均P<0.05);观察组耳鸣、目眩头晕、呕吐恶心、视物旋转等各项症状消失时间均短于对照组(均P<0.05);观察组治疗总有效率及满意度各项评分均高于对照组(均P<0.05)。结论PCIV经六味地黄汤、敏使朗结合治疗效果显著,安全性较高,患者满意度较高,临床症状消失时间更短,血流动力学指标更好。
Objective To observe the effect of Liuwei Dihuang decoction combined with Minzhilang in treating posterior circulation ischemic vertigo(PCIV).Methods A total of 120 cases of PCIV patients who were admitted to our hospital from January 2021 to January 2023 were grouped by double-blind randomization and divided into a control group with 60 cases of MinzhiLang treatment and an observation group with 60 cases of Liuwei Dihuang decoction combined with MinzhiLang treatment,the TCM symptom scores,Dizziness Assessment Rating Scale(DARS)and Dizziness Handicap Inventory(DHI)scores,hemodynamic indexes,time to symptom disappearance,total effective rate of treatment,treatment satisfaction,and incidence of adverse reactions were analyzed in the two groups.Results After the treatment,the observation group's dizziness,deafness and tinnitus,fatigue and tiredness,heaviness of the head,vomiting and nausea,as well as the incidence rates of various Chinese medicine signs and symptoms,DARS scores,DHI scores and adverse reactions,were lower than those of the control group(all P<0.05);The basilar artery(BA),right vertebral artery(RVA),and left vertebral artery(LVA)of the observation group were higher than those of the control group,and the resistance coefficients(RI)were lower(all P<0.05);The disappearance time of tinnitus,dizziness,vomiting and nausea,and visual rotation of the observation group was shorter than that of the control group(all P<0.05);The total effective rate of the observation group's treatments,as well as the scores of the satisfaction level,were higher than those of the control group(all P<0.05).Conclusions PCIV combined with Liuwei Dihuang decoction combined with Minzhilang has a significant therapeutic effect,high safety,high patient satisfaction,shorter clinical symptom duration,and better hemodynamic indicators.
