摘要
目的:基于数据挖掘探讨穴位注射治疗中风后偏瘫的选穴规律,为选穴组方提供参考。方法:检索中国知网、万方数据库、维普中文期刊数据库和Pubmed 2000年1月—2023年5月关于穴位注射治疗中风后偏瘫的相关文献,并建立数据库,利用SPSS Modeler18.0和SPSS Statistics27.0软件对数据进行腧穴频次统计、关联分析以及聚类分析等。结果:最终纳入文献55篇,涉及腧穴86个。曲池、足三里(11.49%、10.70%)使用频率最高。特定穴7种,五输穴使用频率最高。经脉15条,排前3位的依次为手、足阳明经和足少阳经。关联分析示,共16组腧穴组合符合要求,排前3位的依次为曲池-足三里,曲池-肩髃,曲池-肩髃-足三里。高频腧穴聚类分析后得到5组穴位组合;谱系图将排前20位的腧穴分为两类,足三里-曲池为一类,属重点要穴。共28种注射液,丹参注射液、复方丹参注射液、维生素B_(1)和B_(12)注射液使用频率最高。结论:穴位注射治疗中风后偏瘫的经脉循行以阳明经为主,选穴以五输穴为主,多采用足三里和曲池穴,并与近端取穴配伍治疗。
Objective:To explore acupoint selection rules for treating post-stroke hemiplegia with acupoint injection therapy based on data mining,thus to provide reference for acupoint selection and prescription formulation.Methods:The literature related to acupoint injection therapy for post-stroke hemiplegia were retrieved from databases of CNKI,Wanfang,VIP and Pubmed from January 2000 to May 2023.A database was established,and SPSS Modeler 18.0 and SPSS Statistics 27.0 software were utilized for acupoint frequency statistics,correlation analysis and cluster analysis of the data.Results:A total of 55 pieces of literature were included,involving 86 acupoints.The highest frequency of use was observed for Quchi(LI11) and Zusanli(ST36)(11.49% and 10.70%,respectively).Seven specific acupoints were identified,with the highest frequency of use being observed for the five-Shu acupoints.Among the meridians,15 were identified,with the top three being Hand Yangming Meridian,Foot Yangming Meridian and Foot Shaoyang Meridian.Correlation analysis revealed 16 groups of acupoint combinations meeting the criteria,with the top three being LI11-ST36,LI11-Jianyu(LI15),and LI11-LI15-ST36.After high-frequency acupoint clustering analysis,five groups of acupoint combinations were obtained.A dendrogram divided the top 20 acupoints into two categories,with ST36-LI11 forming one category,indicating a focus on key acupoints.A total of 28 types of injection solutions were identified,with the highest frequency of use observed for Danshen injection,Compound Danshen injection,and Vitamin B1/B12 injections.Conclusion:Acupoint injection therapy for post-stroke hemiplegia predominantly follows the course of the Yangming meridians,with the five-Shu points being primarily selected,often utilizing ST36 and LI11,along with proximal acupoints for complementary treatment.
