摘要
目的 探究慢性心力衰竭(CHF)患者自我超越水平及其影响因素。方法 回顾性选取2021年5月至2023年5月郑州大学第一附属医院收治的157例CHF患者,分别采用一般资料调查表、中文版自我超越量表(CSTS)、领悟社会支持量表及Herth希望指数量表调查患者一般资料、自我超越水平、领悟社会支持水平及希望水平,分析CHF患者自我超越水平的影响因素。结果 本次调查共发放157份问卷,有效回收146份,有效回收率为92.99%(146/157)。146例CHF患者CSTS评分为(41.26±7.93)分,其中>45分者40份,占比27.40%。不同病程、心功能分级、受教育程度、月收入、领悟社会支持、希望水平患者CSTS评分比较,差异有统计学意义(P<0.05)。多元线性回归分析显示,心功能分级Ⅲ级是CSTS评分升高的危险因素,大专及以上、月收入>10 000元、高水平领悟社会支持、高度希望水平是CSTS评分升高的保护因素(P<0.05)。结论 CHF患者自我超越水平主要与心功能分级、受教育程度、月收入、领悟社会支持水平、希望水平有关,针对上述因素制定防治措施有助于提高自我超越水平,改善心理状态。
Objective To explore the level of self-transcendence and its influencing factors in patients with chronic heart failure(CHF).Methods A total of 157 patients with CHF admitted to the First Affiliated Hospital of Zhengzhou University from May 2021 to May 2023 were retrospectively selected.General data questionnaire,Chinese self-transcendence scale(CSTS),perceptive social support scale and Herth hope index scale were used to investigate the general information,self-transcendence level,perceptive social support level and hope level of patients,and the influencing factors of self-transcendence level of patients with CHF was analyzed.Results A total of 157 questionnaires were sent out in this survey,and 146 were effectively recovered,with effective recovery rate of 92.99%(146/157).The CSTS score of 146 CHF patients was(41.26±7.93),of which 40 cases were over 45 points,accounting for 27.40%.The score of CSTS in patients with different disease course,cardiac function grade,education level,monthly income,understanding social support and hope level was statistically significant(P<0.05).Multiple linear regression analysis showed that cardiac function gradeⅢwas the risk factor for increasing CSTS score,college degree or above,monthly income>10000 yuan,high level of social understanding and high level of hope were the protective factors for increasing CSTS score(P<0.05).Conclusion The self-transcendence level of CHF patients is mainly related to cardiac function grade,education level,monthly income,understanding social support level and hope level.The prevention and treatment measures based on the above factors are helpful to improve the self-transcendence level and psychological state.
