摘要
目的:系统整理并归纳总结儿童腺样体肥大(AH)的临床组方用药规律及特点,在中医药方面为治疗此病提供选方用药参考。方法:检索国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)自建库至2022年2月中药复方治疗儿童AH全部临床研究文献,建立中药复方治疗儿童AH数据库,采用频次统计分析高频中药及其功效,运用IBM SPSS Modeler 18.0统计软件进行高频药物的关联规则分析,并结合Cytoscape 3.9.1软件完成复杂网络的可视化构建,最后运用IBM SPSS Statistics 25.0统计软件进行高频药物的系统聚类分析。结果:中药频次以甘草、夏枯草、辛夷、苍耳子、浙贝母应用最广。高频药物功效前3名分别为清热药、解表药及补虚药。关联规则分析中支持度>20%的药对有5个,置信度>90%的药对有7个。高频药物当截距为22,聚为5类时效果最好。结论:通过文献研究发现治疗儿童AH药物以清热为基本之法,同时辅以补气、解表、活血及化痰药可增强临床疗效,总结出消瘰丸、六君子汤、苍耳子散合升降散及其他药物组合,提炼其前方用药特点为肺脾同治及脏窍同治,为临床遣方用药提供参考。
Objective:This study aimed to systematically sort out and summarize the clinical prescriptions and characteristics of children with adenoid hypertrophy(AH),and provided a reference for the selection of prescriptions and drugs for the treatment of this disease in traditional Chinese medicine.Methods:To search CNKI and CSPD from the establishment of the database to February 2022,all the clinical research literature on the treatment of children with adenoid hypertrophy by Chinese herbal medicines,this study built a database of Chinese medicines for the treatment of children with AH.The study used frequency statistical analysis of High-frequency traditional Chinese medicine and its efficacy and utilized IBM SPSS Modeler 18.0 to analyze the association rules of high-frequency drugs.It employed Cytoscape 3.9.1 to complete the visual construction of complex networks and finally used IBM SPSS Statistics 25.0 to conduct a systematic cluster analysis of high-frequency drugs.Results:The frequency of traditional Chinese medicine was Glycyrrhizae Radix et Rhizoma,Prunellae Spica,Magnoliae Flos,Xanthii Fructus,and Fritillariae Thunbergii Bulbus.The top three efficacy of high-frequency drugs were heat-clearing drugs,anti-surface drugs,and tonic drugs.In the association rule analysis,there were 5 drug-pair combinations with support>20%and 7 drug-pair combinations with confidence>90%.High-frequency drugs worked best when the intercept was 22 and the clustering was 5.Conclusion:Literature studies showed that the treatment of AH drugs in children was based on clearing fever,supplemented by qi supplementation,releasing exterior,blood activation and phlegm dissolving drugs to enhance the clinical efficacy.This study summarized the combination of Xiaoluo Pill,Liujunzi Decoction,Cangerzi Powder and Shengjiang Powder and other drug combinations,and extracted its previous medication characteristics as the same treatment of lungs and spleen and the same treatment of organs,providing a reference for clinical prescription medication.
