摘要
[目的]探讨内镜下套扎术与常规外科手术治疗轻中度痔疮的临床疗效。[方法]选取入院就诊的轻中度痔疮患者106例,根据随机数表法分为内镜组(53例)和常规手术组(53例),常规手术组患者使用常规手术治疗,内镜组使用内镜下套扎术治疗,回顾性研究比较2组临床疗效,并对2组术中相关指标、术后疼痛评分和水肿评分进行比较,观察2组患者并发症发生情况。[结果]内镜组治疗有效率(94.34%)明显高于常规手术组(77.36%),差异有统计学意义(χ^(2)=6.704,P=0.035)。内镜组手术时间、术中出血量、住院时间、恢复时间和住院费用均明显少于常规手术组,差异有统计学意义(t=13.603、7.676、38.297、19.922、5.609,P=0.000)。内镜组术后疼痛评分和水肿评分均明显低于常规手术组,差异有统计学意义(t=24.962、49.216,P=0.000)。内镜组患者不良反应发生率(13.21%)明显低于常规手术组(41.51%),差异有统计学意义(χ^(2)=10.681,P=0.001)。[结论]相对于常规手术,内镜下套扎术治疗轻中度痔疮的临床疗效显著,可减少术中出血量和住院时间,减轻患者疼痛,且并发症较少,具有较高安全性,值得临床进一步推广。
[Objective]Investigate the clinical efficacy of endoscopic negative pressure ligature and conventional operation on the treatment of mild and moderate hemorrhoids.[Methods]Total 106 cases of patients with mild-to-moderate hemorrhoids,according to the stochastic indicator method,were divided into endoscopic group(53 cases)and normal group(53 cases).Patients in control group were treated with conventional surgery,patients in endoscopy group were treated with endoscopic ligation.The clinical curative effect,the related parameters of intraoperative and postoperative pain score,edema scores,and complications in the two groups were compared.[Results]The effective rate in the endoscopic group(94.34%)was significantly higher than that in the conventional group(77.36%),and the difference was statistically significant(χ~2=6.704,P=0.035).The operative time,intraoperative blood loss,hospitalization time,recovery time and hospitalization cost in the endoscopic group were significantly less than those in the conventional group,with statistically significant differences(t=13.603,7.676,38.297,19.922,5.609,P=0.000).Postoperative pain and edema scores in the endoscopic group were significantly lower than those in the conventional group,with statistically significant differences(t=24.962,49.216,P=0.000).The incidence of adverse reactions in the endoscopic group(13.21%)was significantly lower than that in the conventional group(41.51%),and the difference was statistically significant(χ~2=10.681,P=0.001).[Conclusion]Compared with conventional surgery,endoscopic negative pressure ligation has a significant clinical efficacy in the treatment of mild and moderate hemorrhoids,which can reduce intraoperative blood loss and length of stay,relieve patients'pain,and has fewer complications.It is safe and worthy of further clinical promotion.
