摘要
目的 系统评价补肾类中药复方制剂治疗绝经后骨质疏松症(PMOP)的有效性和安全性。方法 计算机检索中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Cochrane Library、Embase 2007年1月1日-2022年7月31日补肾类中药复方制剂单独服用或联合常规疗法治疗PMOP的随机对照试验(RCT)。采用Cochrane系统评价手册5.1.0进行文献质量评价,运用STATA14.0进行累积Meta分析。结果 共纳入17项RCT,按文献发表年份累积Meta分析提示,补肾类中药复方制剂治疗PMOP于2008年被初次证明有效[OR=2.59,95%CI(1.28,5.23),P<0.05];按样本量大小排序累积Meta分析提示,补肾类中药复方制剂治疗PMOP的OR值相对稳定,其精确性总体呈增高趋势[OR=2.84,95%CI (1.93,4.17),P<0.05],表明治疗组有效率优于对照组。按文献发表年份累积Meta分析提示,补肾类中药复方制剂联合西药治疗PMOP于2011年初次被证明有效[OR=5.63,95%CI(1.55,20.43)P<0.05];按样本量大小排序累积Meta分析提示,补肾类中药复方制剂联合西药治疗PMOP的OR值相对稳定,其精确性总体呈增高趋势[OR=3.49,95%CI(2.26,5.38),P<0.05],提示治疗组明显优于对照组,且时间变化逐渐趋于稳定。Egger检验提示,发表偏倚存在的可能性小。结论 补肾类中药复方制剂治疗PMOP疗效显著,且明显优于单纯西药治疗,值得骨科临床推广使用。
Objective To systematically evaluate the effectiveness and safety of kidney-tonifying TCM compound preparations for the treatment of postmenopausal osteoporosis(PMOP).Methods Randomized controlled trial(RCTs) of kidney-tonifying TCM compound preparations taken alone or combined with conventional therapy for the treatment of PMOP were retrieved from CNKI,Wanfang Data,VIP,CBM,PubMed,Cochrane Library and Embase from 1st,Jan.2007 to 31st,July 2022.Cochrane system evaluator manual 5.1.0 was used for literature quality evaluation,and STATA 14.0 was used for cumulative Meta-analysis.Results A total of 17 RCTs were included.A cumulative Meta-analysis was conducted based on the year of publication in the literature,indicating that kidneytonifying TCM compound preparations were first proven effective in the treatment of PMOP in 2008,with OR=2.59,95% CI(1.28,5.23),P<0.05.A cumulative Meta-analysis was conducted by sorting the sample size,indicating that the OR value of kidney-tonifying TCM compound preparations in the treatment of PMOP was relatively stable,and its accuracy was generally increasing [OR=2.84,95% CI(1.93,4.17),P<0.05],which indicated that the treatment group had a higher effective rate than the control group.According to the cumulative Meta-analysis conducted by the year of publication in the literature,it was suggested that the combination of kidney-tonifying TCM compound preparations and Western medicine was first proven effective in the treatment of PMOP in 2011,with OR=5.63,95%CI(1.55,20.43),P<0.05.The cumulative Meta-analysis conducted in order of sample size showed that the OR value of kidney-tonifying TCM compound preparations combined with Western medicine in the treatment of PMOP was relatively stable,and its accuracy showed an overall increasing trend [OR=3.49,95% CI(2.26,5.38),P<0.05],indicating the treatment group was significantly better than the control group,and the changes over time gradually tended to stabilize.The possibility of publication bias using Egger test was extremely low.Conclusion Kidneytonifying TCM compound preparations have a definite therapeutic effect on PMOP,and are significantly better than simple Western medicine treatment.It is worth promoting and using in orthopedic clinical practice.
