富血小板血浆联合2 940 nm点阵铒激光治疗女性型脱发的临床研究

Clinical Observation of Platelet-rich Plasma Combined with 2940 nm Fractional Erbium Laser In Treatment of Female Pattern Alopecia

摘要 目的:探讨富血小板血浆联合2 940 nm点阵铒激光治疗女性型脱发的临床疗效。方法:选取2022年5月-2023年5月天津市北辰医院皮肤科收治的90例女性型脱发患者分入A组、B组、C组,每组各30例。A组采用富血小板血浆联合2 940 nm点阵铒激光治疗,B组采用米诺地尔联合2 940 nm点阵铒激光治疗,C组采用2 940 nm点阵铒激光治疗。观察记录各组患者临床治疗疗效、满意度、不良反应以及治疗前后主观症状(油腻、瘙痒、头屑、脱发)、毛发镜指标(平均毛发密度、平均毳毛比例、平均毛干直径)情况。结果:A组患者临床治疗有效率(93.33%)明显高于B组(73.33%)、C组(66.67%)(P<0.05)。与治疗前比较,治疗后三组患者油腻、瘙痒、头屑、脱发主观症状评分均降低(P<0.05),且A组油腻、瘙痒、头屑、脱发主观症状评分低于B组、C组(P<0.05)。与治疗前比较,治疗后三组患者毳毛比例均降低,毛发密度、毛干直径均升高(P<0.05),且A组毳毛比例低于B组、C组,毛发密度、毛干直径高于B组、C组(P<0.05)。A组患者治疗后的总满意度(96.67%)明显高于B组(76.67%)、C组(73.33%)(P<0.05),但三组患者不良反应比较差异无统计学意义(P>0.05)。结论:富血小板血浆联合2 940 nm点阵铒激光治疗女性型脱发能取得较佳的临床疗效,可改善患者的头皮症状,增加毛发密度、毛干直径,同时安全性高,值得临床推广。 Objective To explore the clinical effect of platelet-rich plasma combined with 2940 nm fractional erbium Laser in the treatment of female pattern alopecia.Methods From May 2022 to May 2023,90 female patients with alopecia who were admitted to the dermatology department of Tianjin Beichen Hospital were selected and divided into Group A,Group B,and Group C,with 30 patients in each group.Group A was treated with platelet-rich plasma combined with 2940 nm fractional erbium laser therapy,Group B was treated with minoxidil combined with 2940 nm fractional erbium laser therapy,and Group C was treated with only 2940 nm fractional erbium laser therapy.The clinical efficacy,satisfaction,adverse reactions,subjective symptoms before and after treatment(oiliness,itching,dandruff,and hair loss),and trichoscope indicators(average hair density,average vellus hair ratio,average hair shaft diameter)of patients in each group were observed and recorded.Results The clinical treatment efficiency of Group A patients(93.33%)was significantly higher than that of Group B(73.33%)and Group C(66.67%)(P<0.05).Compared to pre-treatment,the scores of subjective symptoms such as oiliness,itching,dandruff,and alopecia were reduced in all three groups(A,B,and C)after treatment(P<0.05),and the scores of these symptoms in Group A were lower than those in Groups B and C(P<0.05).Compared to pre-treatment,the proportion of vellus hairs decreased,while hair density and hair shaft diameter increased in all three groups after treatment(P<0.05).Moreover,Group A had a lower proportion of vellus hairs and higher hair density and hair shaft diameter compared to Groups B and C(P<0.05).The overall satisfaction of Group A patients after treatment(96.67%)was significantly higher than that of Groups B(76.67%)and C(73.33%)(P<0.05).However,there was no statistically significant difference in adverse reactions among the three groups(P>0.05).Conclusion The combination of platelet-rich plasma and 2940 nm fractional erbium laser therapy for female alopecia can achieve better clinical efficacy,improve scalp symptoms,increase hair density and hair shaft diameter,and has high safety.It is worthy of clinical promotion.
