摘要
目的分析中医扶正培本辅助腹腔镜结直肠癌手术对老年患者免疫功能及miR-34a、ZEB2表达的影响。方法将2020年1月—2022年1月医院收治的100例老年结直肠癌患者随机分为观察组及对照组。对照组采用腹腔镜根治术进行治疗,观察组在对照组基础上采用中医扶正培本方案干预;调查患者排气用时、腹胀持续用时及血中胃动素和胃泌素水平;治疗前及术后6个月时检测患者T细胞亚群及病灶和术区miR-34a及ZEB2水平。结果观察组腹胀持续时间、排气时间均显著低于对照组(P<0.05),且观察组血中胃动素及胃泌素水平均显著高于对照组(P<0.05);观察组术后1年转移率及复发率明显低于对照组,且差异有统计学意义(P<0.05);治疗后观察组血中CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)及自然杀伤细胞(NK细胞)水平显著高于对照组(P<0.05),血中CD_(8)^(+)水平显著低于对照组(P<0.05);治疗后观察组组织中miR-34a显著高于对照组(P<0.05),ZEB2水平显著低于对照组(P<0.05)。结论采用中医扶正培本辅助腹腔镜对老年结直肠癌患者进行治疗后,可显著提高患者免疫功能并提高miR-34a、ZEB2水平。
Objective To analyze the effect of reinforcing healthy Qi and root method assisting laparoscopic colorectal cancer surgery on immune function and expression of miR-34a and ZEB2 in elderly patients.Methods A hundred elderly patients with colorectal cancer admitted to our hospital from January 2020 to January 2022 were randomly divided into observation group and control group.The control group was treated with laparoscopic radical operation,while the observation group was treated with traditional Chinese medicine reinforcing healthy Qi and root method on the basis of the control group.The patients′exhaust time,duration of abdominal distention and serum motilin and gastrin levels were investigated before and 6 months after treatment.T cell subsets and levels of miR-34a and ZEB2 in lesions and surgical areas of patients.Results The duration of abdominal distension and exhaust time in the observation group were significantly lower than those in the control group(P<0.05),and the levels of motilin and gastrin in the blood of the observation group were significantly higher than those in the control group(P<0.05).The metastasis rate and recurrence rate in the observation group were significantly lower than those in the control group one year after operation,and the difference was statistically significant(P<0.05).After treatment,the levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)and natural killer cells(NK cells)in the observation group were significantly in the control group(P<0.05),the level of CD_(8)^(+)in blood was significantly lower than that in the control group(P<0.05).After treatment,the level of miR-34a in the observation group was significantly higher than that in the control group(P<0.05),and the level of ZEB2 in the observation group was significantly lower than that in the control group(P<0.05).Conclusions The use of traditional Chinese medicine reinforcing healthy Qi and root method assisting laparoscopy in the treatment of elderly patients with colorectal cancer can significantly improve the immune function of patients and increase the levels of miR-34a and ZEB2.
