摘要
目的探讨认知行为干预联合音乐放松训练对主动脉夹层术后心脏肌力收缩储备及认知状态的影响。方法选取2020年9月-2023年3月期间入院的主动脉夹层患者90例。按随机数字表法分为观察组和对照组,每组45例,对照组给予常规护理模式,观察组在对照组基础上行认知行为干预联合音乐放松训练,比较两组干预后的心脏肌力收缩储备指标[左室射血分数(LVEF)、左心室收缩末期内径(LVEDV)、一秒率(FEV1/FVC)]、肺功能恢复情况[氧分压(PO_(2))、血氧饱和度(SaO_(2))、氧合指数(OI)]、认知功能评分、心理状态评分、疼痛评分、生活质量评分。结果干预后组间比较,观察组LVEF、LVEDV、FEV1/FVC、PO_(2)、OI水平明显高于对照组(P<0.05),SaO_(2)水平明显低于对照组(P<0.05),观察组认知功能和生活质量评分明显高于对照组(P<0.05),疼痛评分、焦虑、抑郁评分均明显低于对照组(P<0.05)。结论主动脉夹层术后患者采用认知行为干预联合音乐放松训练后,能够提高患者的心脏肌力收缩储备和认知状态,其负性情绪和生活质量得到改善,值得临床推广应用。
Objective To explore the effect of cognitive behavioral intervention combined with music relaxation training on cardiac contractile reserve and cognitive status after Aortic dissection.Methods 90 patients with aortic coarctation who were admitted to the hospital between September 2020 and March 2023 were selected and divided into the observation group and the control group according to the randomized numerical table method,with 45 cases in each group.The control group was given routine nursing mode,and the observation group was given cognitive behavioral intervention combined with music relaxation training on the basis of the control group.After the intervention,the two groups were compared in terms of cardiac muscle contractile reserve indexes[left ventricular ejection fraction(LVEF),left ventricular end-systolic internal diameter(LVEDV),one-second rate(FEV1/FVC)],pulmonary function recovery[partial pressure of oxygen(PO_(2)),oxygen saturation(SaO_(2)),oxygenation index(OI)],cognitive function scores,psychological state scores,pain scores,and quality of life scores.Result In the post-intervention intergroup comparison,the levels of LVEF,LVEDV,FEV1/FVC,PO_(2),and OI in the observation group were significantly higher than those in the control group(P<0.05),and the level of SaO_(2) was significantly lower than that in the control group(P<0.05).The cognitive function and quality of life scores of the observation group were significantly higher than those of the control group(P<0.05),and pain scores,anxiety and depression scores were significantly lower than those of the control group(P<0.05).Conclusion Cognitive behavioral intervention combined with music relaxation training can improve the cardiac contractile reserve and cognitive status of patients after Aortic dissection,and their negative emotions and quality of life can be improved,which is worthy of clinical application.
