摘要
目的探讨达格列净辅助治疗早期2型糖尿病肾病的临床效果及对患者氧化应激和血管内皮功能的影响。方法随机对照研究。抽取2021年9月至2023年1月就诊于临沂市中医医院的2型糖尿病肾病患者108例,采用随机数字表法将纳入患者分为对照组与观察组,每组54例。对照组接受常规药物治疗,观察组在常规药物治疗的基础上加用达格列净辅助治疗,两组均治疗2个月。比较两组临床疗效、血糖水平[餐后2 h血糖(2hPBG)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)]、血管内皮功能[血管性血友病因子(vWF)、人内皮素(ET-1)]及氧化应激因子水平[超氧化物歧化酶(SOD)、丙二醛(MDA)]。结果观察组总有效率(96.30%,52/54)高于对照组(81.48%,44/54),P<0.05。治疗后,观察组FPG、2hPBG、HbA1c水平低于对照组(P<0.05);治疗后,观察组vWF、ET-1水平低于对照组(P<0.05);治疗后,观察组SOD水平高于对照组,MDA水平低于对照组(P<0.05)。结论采用达格列净辅助治疗早期2型糖尿病肾病患者效果显著,患者氧化应激反应、血糖水平以及血管内皮功均能得到明显改善。
Objective To investigate the effect of dapagliflozin in adjuvant treatment of early type 2 diabetic nephropathy,and its influence on oxidative stress and vascular endothelial function.Methods A total of 108 patients with type 2 diabetic nephropathy admitted to Linyi Traditional Chinese Medicine Hospital from September 2021 to January 2023 were selected for the randomized controlled study.And the patients were divided into the control group and the observation group by random number table method,with 54 cases in each group.The control group received conventional drug treatment,while the observation group received adjuvant treatment with dapagliflozin based on the conventional drug treatment.Both groups were treated for 2 months.The clinical efficacy,levels of blood glucose evaluated by 2-hour postprandial blood glucose(2hPBG),fasting plasma glucose(FPG)and hemoglobin A1c(HbA1c),vascular endothelial function assessed by von willebrand factor(vWF)and endothelin(ET-1),and levels of oxidative stress factors,including superoxide dismutase(SOD)and malondialdehyde(MDA),were compared between the two groups.Results The total effective rate of the observation group(96.30%,52/54)was higher than that of the control group(81.48%,44/54),P<0.05.After treatment,the levels of FPG,2hPBG and HbA1c in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of vWF and ET-1 in the observation group were lower than those in the control group(P<0.05).After treatment,the observation group had higher SOD level and lower MDA level compared with the control group(P<0.05).Conclusions Dapagliflozin as adjuvant treatment has a significant therapeutic effect on early type 2 diabetic nephropathy,which can significantly improve the oxidative stress reaction,blood glucose level and vascular endothelial function.
