摘要
目的研究肩胛骨骨折治疗行经Judet和肩袖肌间隙入路的疗效差异。方法便利选择徐州仁慈医院于2021年1月—2023年6月收治的78例肩胛骨骨折患者为研究对象,采用随机数表法分为两组,对照组行Judet入路(39例),观察组行肩袖肌间隙入路(39例)。比较两组手术指标、肩关节功能、肩胛骨恢复优良率及并发症。结果两组骨折愈合用时对比,差异无统计学意义(P>0.05);观察组术中出血量、切口长度、术后引流量、手术耗时、术后消肿用时均优于对照组,差异有统计学意义(P均<0.05)。观察组各项肩关节功能(疼痛、日常活动、肌力、肩关节活动度)评分高于对照组,差异有统计学意义(t=4.628、7.230、8.107、4.406,P均<0.05)。观察组肩胛骨恢复优良率(97.44%)高于对照组(79.49%),差异有统计学意义(χ^(2)=4.522,P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论较Judet入路,经肩袖肌间隙入路治疗肩胛骨骨折切口更小,出血、引流更少,恢复更快,并发症更少,术后肩关节功能恢复更好,疗效更佳。
Objective To study the difference in the efficacy of trans-Judet and trans-rotator cuff muscle gap approaches in the treatment of scapula fracture.Methods A total of 78 patients with scapular fractures admitted to Xuzhou Renci Hospital from January 2021 to June 2023 were conveniently selected as the research objects.They were divided into two groups by random number table method.The control group was treated with Judet approach(39 cases),and the observation group was treated with rotator cuff intermuscular approach(39 cases).The surgical indexes,shoulder joint function,excellent and good rate of scapular recovery and complications were compared between the two groups.Results There was no significant difference in fracture healing time between the two groups(P>0.05);the intraoperative blood loss,incision length,postoperative drainage volume,operation time and postoperative swelling time in the observation group were better than those in the control group,the differences were statistically significant(all P<0.05).The score of shoulder joint function(pain,daily activity,muscle strength,shoulder joint activity)in the observation group was higher than that in the control group,and the differences were statistically significant(t=4.628,7.230,8.107,4.406,all P<0.05).The excellent and good rate of scapula recovery in the observation group(97.44%)was higher than that in the control group(79.49%),and the difference was statistically significant(χ^(2)=4.522,P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Compared with Judet approach,trans-rotator cuff muscle gap approach for scapula fracture has smaller incision,less bleeding and drainage,faster recovery,fewer complications,better postoperative recovery of shoulder joint function,and better efficacy.
