摘要
目的观察复方紫草油膏对湿热下注型肛瘘术后患者炎症指标和相关生长因子的影响。方法将114例湿热下注型肛瘘术后患者按照随机数字表法分为2组,对照组57例予湿润烧伤膏治疗,治疗组57例在对照组治疗基础上加复方紫草油膏治疗。2组均治疗14天。比较2组疗效;比较2组治疗前后中医证候评分、炎症因子水平、生长因子水平变化;比较2组治疗安全性。结果治疗组总有效率92.98%(53/57),对照组总有效率77.19%(44/57),治疗组疗效优于对照组(P<0.05)。2组治疗后各项中医证候评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后淀粉样蛋白A(SAA)、白细胞介素14(IL-14)、Toll样受体9(TLR9)水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、转化生长因子β_(1)(TGF-β_(1))水平均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论复方紫草油膏治疗湿热下注型肛瘘术后患者效果良好,可改善患者主要中医证候,抑制炎性反应,上调生长因子水平,治疗安全性良好。
Objective To study the effect of Compound Zicao Oil on inflammatory reaction and growth factors in postoperative patients with anal fistula of damp-heat infusion type.Methods A total of 114 patients with anal fistula of damp-heat injection type were randomly divided into control group(n=57)and observation group(n=57).All patients were treated with burn ointment treatment,those in the observation group were additionally treated with Compound Zicao Oil.After 14 days of treatment,the clinical efficacy,traditional Chinese medicine(TCM)syndrome score,inflammatory factors,growth factor level,and treatment safety between groups were evaluated.Results The total effective rate of the observation group was higher than that of the control group(92.98%[53/57]vs 77.19%[44/57],P<0.05).After treatment,the TCM symptom scores in the both groups were lower than those before treatment(P<0.05),which in the observation group were more common relative to the control group(P<0.05).After treatment,the levels of serum amyloid A(SAA),interleukin-14(IL-14)and Toll-like receptor 9(TLR-9)in the both groups were lower than those before treatment(P<0.05),which in the observation group were more pronounced compared to the control group(P<0.05).After treatment,vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and transforming growth factor-β_(1)(TGF-β_(1))in the both groups were higher than those before treatment(P<0.05),which in the observation group were more prevalent compared with the control group(P<0.05).During the treatment,the incidence of adverse reactions in the two groups was similar(P>0.05).Conclusion Compound Zicao Oil has a good therapeutic effect in patients with anal fistula of damp-heat infusion type,which can improve the main TCM symptom,and it can inhibit inflammatory reaction rate,increase growth factor level,and it has good therapeutic safety.
