摘要
目的探讨儿童鼻-鼻窦炎患儿实施鼻腔冲洗治疗的临床价值。方法于2021年3月-2023年9月期间采集病例建档,对象为四会市人民医院诊治的儿童鼻-鼻窦炎患儿共96例,均实施鼻腔冲洗,根据使用鼻腔冲洗液的不同分为三组,即对照组(应用0.9%生理盐水)、治疗组1(应用布地奈德+生理盐水)、治疗组2(应用2.3%高渗海盐水);观察对比三组的临床症状缓解时间、生活质量、鼻内镜MLMES评分、不良反应发生率。结果治疗组1的鼻涕减少、鼻塞、头痛和咳嗽缓解时间均较对照组、治疗组2明显缩短(P<0.05);治疗前,三组MLMES评分对比,无明显差异(P>0.05),治疗1周、2周,三组评分均低于治疗前,且治疗组1的MLMES评分显著低于对照组、治疗组2(P<0.05);三组不良反应发生率对比,无明显差异(P>0.05);治疗前,三组SF-36评分对比,无明显差异(P>0.05),治疗后,三组SF-36评分均不同程度升高,且治疗组1的评分显著高于对照组、治疗组2(P<0.05)。结论儿童鼻-鼻窦炎应用生理盐水+布地奈德鼻腔冲洗,相较于单纯盐水冲洗,可增强疗效,促进患儿症状改善。
Objective To explore the clinical value of nasal irrigation treatment in children with rhinosinusitis.Methods From March 2021 to September 2023,a total of 96 children with rhinosinusitis diagnosed and treated at the People's Hospital of Sihui City were collected for case filing.All patients underwent nasal irrigation and were divided into three groups based on the use of nasal irrigation solution:control group(using 0.9%physiological saline),treatment group 1(using budesonide+physiological saline),and treatment group 2(using 2.3%hypertonic saline);observe and compare the clinical symptom relief time,quality of life,nasal endoscopy MLMES score,and incidence of adverse reactions among the three groups.Results The relief time of nasal discharge,nasal congestion,headache,and cough in treatment group 1 was significantly shorter than that in control group and treatment group 2(P<0.05);before treatment,the MLMES scores of the three groups were compared(P>0.05).After 1 and 2 weeks of treatment,the MLMES scores of the three groups were lower than before treatment,and the MLMES scores of treatment group 1 were significantly lower than those of the control group and treatment group 2(P<0.05);there was no significant difference in the incidence of adverse reactions among the three groups(P>0.05);before treatment,the SF-36 scores of the three groups were compared(P>0.05).After treatment,the SF-36 scores of the three groups all increased to varying degrees,and the scores of treatment group 1 were significantly higher than those of the control group and treatment group 2(P<0.05).Conclusion The application of physiological saline and budesonide nasal irrigation in children with rhinosinusitis can enhance the therapeutic effect and promote the improvement of symptoms compared to simple saline irrigation.
