摘要
肠及肠系膜损伤占钝性腹部损伤的3%~5%,常发生在交通事故及其他碰撞之后,是仅次于肝脾损伤的腹部钝性创伤形式。单纯肠及肠系膜损伤患者早期症状较轻且不典型,主要表现为失血、心率增快、持续性腹痛及恶心呕吐,影像学表现细微且隐匿,容易引起病情延误甚至漏诊。本文报道了一例车祸外伤致肠系膜及肠系膜上动脉分支损伤的失血性休克患者,在介入手术围术期出现心脏骤停,经积极抢救后复苏成功,最终康复出院。
Bowel and mesenteric injuries(BMIs)account for 3%~5%of blunt abdominal injuries,which often occur after traffic accidents and other collisions.It is a form of blunt abdominal trauma second only to liver and spleen injuries.The early symptoms of patients with simple BMIs were mild and atypical,such as blood loss,increased heart rate,persistent abdominal pain,nausea and vomiting.The imaging findings were subtle and hidden,which could easily lead to delay or even missed diagnosis.This article reports a case of early hemorrhagic shock caused by injury of mesenteric and superior mesenteric artery branches after traffic accident.The patient suffered from cardiac arrest during the perioperative period of interventional operation,recovered successfully after active rescue,and finally recovered and discharged from the hospital.
