摘要
目的探讨超声引导下肋间神经射频热凝联合臭氧注射治疗胸背部带状疱疹后神经痛的临床疗效。方法回顾性分析2021年10月至2023年5月,江南大学附属医院、江南大学附属妇产医院疼痛科收治住院的胸背部带状疱疹后神经痛患者,经筛选符合纳入标准的60例。按照治疗方式不同分为超声引导下肋间神经射频热凝组(A组)和超声引导下肋间神经射频热凝射频联合臭氧注射组(B组),每组各30例。观察治疗前及治疗后1 d、1周、1个月及3个月的数字等级评分(NRS)和匹兹堡睡眠质量指数(PSQI)以及治疗后3个月的疼痛缓解率,比较两组指标的统计学差异。结果比较治疗后1 d、1周、1个月及3个月的两组NRS评分及PSQI,B组均优于A组,差异均有统计学意义(P均<0.05)。治疗3个月后两组疼痛缓解率,B组较A组改善明显,差异有统计学意义(χ^(2)=4.71,P=0.030)。两组患者均未见气胸、呼吸困难、血气胸及出血等并发症。结论超声引导下肋间神经射频热凝联合臭氧注射治疗胸背部带状疱后神经痛,能够更有效迅速的缓解疼痛、控制病情,改善预后。
Objective To investigate the clinical efficacy of ultrasound-guided radiofrequency thermocoagulation combined with ozone injection on intercostal nerve in patients with postherpetic neuralgia.Methods From October 2021 to May 2023,60 hospitalized patients with postherpetic neuralgia were retrospectively analyzed.The patients were divided into two groups according to the therapy:group A(ultrasound-guided radiofrequency thermocoagulation on intercostal nerve group)and group B(ultrasound-guided radiofrequency thermocoagulation combined with ozone injection on intercostal nerve group),30 cases in each group.Number rating scale(NRS)and Pittsburgh sleep quality index(PSQI)were measured before the treatment and 1 d,1 week,1 month and 3 months after the treatment.The pain relief rate was observed at 3 months after the treatment,and the statistical differences were compared between the two groups.Results The NRS and PSQI were better in group B than those in group A at 1 d,1 week,1 month and 3 months after the treatment(all P<0.05).The pain relief rate was significantly higher in group B than that in group A at 3 months after the treatment(χ^(2)=4.71,P=0.030).No complications were found,such as pneumothorax,dyspnea,hemopneumothorax and bleeding occurred in both groups.Conclusion Ultrasound-guided radiofrequency thermocoagulation combined with ozone injection on intercostal nerve can effectively and rapidly relieve pain and improve the prognosis in patients with postherpetic neuralgia.
