摘要
目的探讨骨性关节炎(osteoarthritis,OA)采用改良外侧斜行小切口全髋关节置换术的疗效及对髋关节功能的影响。方法选取2022年5月—2023年5月郑州大学第一附属医院收治的髋关节OA患者96例,按采用手术方法不同分组,对照组48例应用标准后外侧Gibson入路关节置换术,研究组48例应用改良外侧斜行小切口关节置换术,比较两组围术期各指标、髋关节功能、炎性因子及术后并发症。结果术前两组Harris评分比较无统计学意义(P>0.05),术后1个月和6个月两组Harris评分均明显上升,研究组术后1个月评分(90.08±3.54)分,高于对照组的(86.84±3.48)分(P<0.05),研究组术后6个月Harris评分(94.72±4.53)分,与对照组(93.17±4.39)分比较无统计学意义(P>0.05);研究组切口长度(7.97±0.36)cm,明显短于对照组的(11.25±0.47)cm(P<0.05),两组术中的失血量及手术时长比较无统计学意义(P>0.05);两组术前CRP、IL-6水平比较无意义(P>0.05),术后1 d和3 d均有上升,但研究组CRP、IL-6水平明显低于对照组(P<0.05);研究组并发症发生率(4.17%)低于对照组的(18.75%)(P<0.05)。结论改良外侧斜行小切口THR能明显改善OA患者髋关节功能,缩短切口的长度,缓解应激反应,提高治疗的安全性。
Objective To investigate the effect of modified lateral oblique small incision total hip arthroplasty on hip function in osteoarthritis(OA).Methods Totally ninety-six patients with hip OA admitted to the First Affiliated Hospital of Zhengzhou University from May 2022 to May 2023 were divided into different groups according to different surgical methods.Standard posterolateral Gibson approach joint replacement was applied in 48 cases in the control group,and modified lateral oblique small incision joint replacement was applied in 48 cases in the study group.Perioperative indexes,hip function,inflammatory factors and postoperative complications were compared between the two groups.Results There was no statistical significance in the Harris score between the two groups before surgery(P>0.05),and the Harris score of the two groups increased significantly 1 month and 6 months after surgery.The score of the study group at 1 month after surgery was(90.08±3.54)points,which was higher than that of the control group(86.84±3.48)points(P<0.05).Six months after operation,Harris score in the study group(94.72±4.53)was not significantly higher than that of control group(93.17±4.39)(P>0.05).The incision length of the study group was(7.97±0.36)cm,which was significantly shorter than that of the control group(11.25±0.47)cm(P<0.05),and there was no statistical significance in the amount of blood loss and operation duration between the two groups(P>0.05).There was no statistical significance in the levels of CRP and IL-6 between the two groups before surgery(P>0.05),but the levels of CRP and IL-6 in the study group were significantly lower than those in the control group(P<0.05).The complication rate of the study group(4.17%)was lower than that of the control group(18.75%)(P<0.05).Conclusion Modified lateral oblique small incision THR can significantly improve the hip function of OA patients,shorten the length of incision,relieve stress response,and improve the safety of treatment.
