男性体检人群谷氨酰转肽酶与代谢性疾病的相关性研究

Correlation between glutamyl transpeptidase and metabolic diseases in men receiving health check-up

摘要 目的基于横断面人群研究男性谷氨酰转肽酶(GGT)与6种常见代谢性疾病的相关性,为代谢性疾病的防治提供依据。方法以郑州市某医院2021年25529名男性健康体检者为研究对象,使用R corrplot包分析GGT与其他代谢指标的相关性,采用多因素logistic回归分析GGT不同水平组(五分位分组,从低到高依次为Q1~Q5组)与代谢性疾病的关联,并运用限制性立方样条(RCS)深入研究GGT与单一代谢性疾病的剂量-反应关系。结果GGT、腰围、收缩压、舒张压、体重指数(BMI)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、血尿酸、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)和三酰甘油葡萄糖指数(TyG)随着代谢性疾病数量的增加而升高,高密度脂蛋白胆固醇(HDL-C)却相反;与Q1组(1~16 U/L)相比,Q3组(23~29 U/L)、Q4组(30~44 U/L)和Q5组(≥45 U/L)6种代谢性疾病的患病风险均增加(均P<0.05),模型3中Q4组(正常范围GGT水平)高尿酸血症、血脂异常和脂肪肝的OR值(95%CI)分别为1.82(1.61~2.06)、2.53(2.31~2.76)和2.00(1.80~2.22)。单一代谢性疾病研究中RCS结果显示GGT仅与肥胖、高尿酸血症、血脂异常及脂肪肝风险呈非线性关联关系(均P<0.001)。结论GGT可能直接或通过尿酸和(或)脂代谢间接参与代谢调节。正常范围内GGT水平的升高与6种常见代谢性疾病风险相关,建议男性GGT控制在23 U/L以下,以预防代谢性疾病的发生。 Objective To explore the correlation between glutamyl transpeptidase(GGT)level and six common metabolic diseases in men based on cross-sectional population to provide evidence for the prevention and treatment of metabolic diseases.Methods A total of 25529 male participants from a hospital in Zhengzhou in 2021 who underwent health check-up were enrolled.R corrplot was used to analyze the correlation between GGT and other metabolic indexes,and multivariate logistic regression was used to analyze the correlation between different GGT levels(grouped into quintiles,Q1 to Q5,from low to high)and metabolic diseases.The dose-response relationship between GGT and single metabolic disease was further studied by using restricted cubic splines(RCS).Results The levels of GGT,waist circumference,systolic blood pressure,diastolic blood pressure,body mass index(BMI),alanine aminotransferase(ALT),aspartate aminotransferase(AST),serum uric acid,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG)and triglyceride-glucose index(TyG)increased with the increase of the number of metabolic diseases,while high density lipoprotein cholesterol(HDL-C)exhibited an inverse relationship.The risks of six metabolic diseases were increased in Q3(23-29 U/L),Q4(30-44 U/L)and Q5(≥45 U/L)groups compared with Q1 group(1-16 U/L)(all P<0.05).The odds ratios(95%CI)of hyperuricemia,dyslipidemia,and fatty liver in model 3 of group Q4(normal GGT level)were 1.82(1.61-2.06),2.53(2.31-2.76),and 2.00(1.80-2.22),respectively.In the individual metabolic disorder study,the RCS analysis revealed that GGT exhibited a non-linear association only with the risks of obesity,hyperuricemia,dyslipidemia,and fatty liver(all P<0.001).Conclusion GGT may be involved in metabolic regulation directly or indirectly through uric acid and/or lipid metabolism.Elevated levels of GGT within the normal range were associated with the risk of six common metabolic diseases.It is recommended to maintain GGT levels in males below 23 U/L to prevent the occurrence of metabolic diseases.
