摘要
目的:探讨多层螺旋CT(MSCT)检查在肺磨玻璃结节(GGN)定性诊断中的临床价值。方法:选取2019年1月~2021年12月本院收治的100例GGN患者作为研究对象,均行胸部X射线、MSCT检查,并以手术病理为金标准,统计GGN良恶性情况及诊断价值,并采用Kappa检验各方法定性诊断与金标准的一致性;比较良恶性GGN在毛刺征、分叶征、空泡征、血管集束征、胸膜凹陷征等特征上的差异。结果:100例GGN经金标准检出68例恶性;其中胸部X射线检出63例,阳性率为92.65%;MSCT检出67例,阳性率为98.53%;相比胸部X射线,MSCT定性诊断GGN的灵敏度、准确度、阴性预测值更高(P<0.05);Kappa检验显示,胸部X射线与金标准Kappa值=0.713,一致性尚可;MSCT与金标准Kappa值=0.932,一致性极好;恶性组毛刺征、分叶征、空泡征、血管集束征、胸膜凹陷征占比高于良性组(P<0.05)。结论:MSCT在GGN定性诊断中价值高,便于早期识别良恶性,以实现针对性治疗,更好改善患者预后。
Objective:To investigate the clinical value of multi-layer spiral CT(MSCT)examination in the qualitative diagnosis of pulmonary ground glass nodules(GGN).Methods:100 patients with GGN admitted to our hospital from January 2019 to December 2021 were selected as the study subjects,all of whom underwent chest X-ray and MSCT examinations.Surgical pathology was used as the gold standard,and the benign and malignant status and diagnostic value of GGN were statistically analyzed.Kappa test was used to verify the consistency between qualitative diagnosis and gold standard;Compare the differences between benign and malignant GGNs in features such as spiculation sign,lobulation sign,vacuole sign,vascular bundle sign,and pleural indentation sign.Results:100 GGN detected 68 malignant by gold standard;Among them,63 chest X-ray,the positive rate was 92.65%;In 67 cases were detected by MSCT,the positive rate was 98.53%;Compared to the chest X-ray,the sensitivity,accuracy and negative predictive value of MSCT to qualitatively diagnose GGN were higher(P<0.05);The Kappa test showed,chest X-ray and gold standard Kappa value=0.713,the consistency is acceptable;MSCT with the gold standard Kappa value=0.932,excellent consistency;The proportion of burr,leaf segmentation,vacuolation,vascular collection and pleural depression in the malignant group was higher than that in the benign group(P<0.05).Conclusion:MSCT has high value in qualitative diagnosis of GGN and facilitates early identification of benign and malignant patients to achieve targeted treatment and better improve patient prognosis.
