摘要
目的观察“调督安神”针刺联合四逆散治疗老年顽固性失眠患者的疗效,为其治疗提供方法。方法纳入2021年6月—2023年6月在遂宁市第三人民医院治疗的中医辨证为心脾两虚证的老年顽固性失眠患者102例,采用随机数字表法分为对照组和观察组,每组51例。对照组口服右佐匹克隆片,观察组在与对照组相同治疗的基础上加用“调督安神”针刺联合四逆散治疗,2组患者均持续治疗2周。比较2组临床疗效、中医证候评分和匹兹堡睡眠质量指数量表(PSQI)评分。结果观察组治疗有效率高于对照组(94.12%vs 78.43%,P<0.05)。治疗1周后,观察组患者神疲乏力、多梦易醒、头晕目眩、心悸易惊及注意力不集中评分均低于同组治疗前(P<0.05),而对照组上述各项评分与治疗前差异均无统计学意义(P>0.05);治疗2周后,两组的神疲乏力、多梦易醒、头晕目眩、心悸易惊及注意力不集中评分均低于同组治疗前,且观察组上述评分低于对照组(均P<0.05)。治疗1周后,观察组患者PSQI评分明显低于同组治疗前和对照组(均P<0.05);治疗2周后,观察组和对照组患者PSQI评分明显低于同组治疗前,且观察组PSQI评分低于对照组(均P<0.05)。结论西药右佐匹克隆片联合“调督安神”针刺与四逆散加味方治疗老年顽固性失眠患者,疗效确切,可显著改善患者的临床症状,具有一定的应用价值。
Objective To observe the curative effects of“Tiaodu Anshen”acupuncture combined with Sini powder in the treatment of elderly patients with intractable insomnia,and provide a method for its treatment.Methods A total of 102 elderly patients with intractable insomnia treated in Third People's Hospital of Suining City from June 2021 to June 2023 were included in the study.They were divided into control group and observation group by random number table method,with 51 cases in each group.The control group took dexzopiclone tablet orally,and the observation group was treated with“Tiaodu Anshen”acupuncture combined with Sini powder on the basis of the same treatment as the control group.Both groups were treated for 2 weeks.The clinical efficacy,traditional Chinese medicine(TCM)syndrome score and Pittsburgh Sleep Quality Index Scale(PSQI)score were compared between the two groups.Results The effective rate of treatment in the observation group was higher than that in the control group(94.12%vs 78.43%,P<0.05).After 1 week of intervention,the scores of fatigue,dreaminess,dizziness,palpitation and inattention of the observation group were significantly lower than those of the same group before intervention(P<0.05),while the control group had no statistically significant differences in the above scores before and after treatment(P>0.05).After 2 weeks of intervention,the scores of fatigue,dreaminess,dizziness,palpitation and inattention of both groups were significantly lower than those of the same group before intervention,and the scores of the observation group were lower than those of the control group(P<0.05).After 1 week of treatment,PSQI score of the observation group was significantly lower than that of the same group before treatment and the control group(all P<0.05).After 2 weeks of treatment,PSQI scores of both groups were significantly lower than those of the same group before intervention,and the score of the observation group was lower than that of the control group(all P<0.05).Conclusion The combination of dexzopiclone tablet and“Tiaodu Anshen”acupuncture combined with Sini powder has a definite therapeutic effect on elderly patients with intractable insomnia.It can significantly improve the clinical symptoms of patients and has certain application value.
