摘要
良性前列腺增生症(Benign prostatic hyperplasia,BPH)临床以排尿困难为主要表现,近年来发病率日趋上升。中医学认为BPH多与肾虚相关,郭军教授在此基础上结合古籍文献研究及多年临床经验提出“脑-心-肾-精室”轴理论,认为心与脑同居上焦,藏神而调节肾与精室生理功能,肾与精室同居下焦,主水而直接影响尿液代谢,“脑-心-肾-精室”轴由上及下,环环相扣,从整体调控尿液的代谢。“脑-心-肾-精室”轴紊乱则会导致BPH发生。BPH病机包括脑心失养、神失所主,肾虚不固、气化失司,邪阻下焦、精室失用三类,临床治疗以调心益脑、益肾填精、通利精室取得了良好的效果。文中通过系统阐释调心益脑、益肾填精、通利精室在BPH治疗中的内在分子机制变化,可为临床应用“脑-心-肾-精室”轴理论干预BPH的生物学基础研究提供依据。
Benign prostatic hyperplasia(BPH)is characterized by difficulty in urination and its incidence has been increasing in recent years.According to traditional Chinese medicine(TCM),BPH is mainly related to kidney deficiency.On the basis of textual research and years of clinical experience,Professor Guo Jun puts forward the theory of“brain-heart-kidney-semen chamber”axis.He believes that the heart and brain in the upper energizer have the functions of storing spirit and regulating the physiological activities of the kidney and semen chamber.The kidneys and the semen chamber in the lower energizer govern the water flow and directly affect the metabolism of urine.The“brain-heart-kidney-semen chamber”axis connecting the upper and lower energizer can thus regulate the metabolism of urine as a whole.Disturbances in this axis can lead to BPH.The basic mechanisms of BPH include brain and heart dysfunctions,failure of spirit in governing,kidney deficiency,dysfunction of qi transformation,pathogen obstructing lower energizer,and dysfunction of semen chamber.Accordingly,the clinical treatment of this disease should adopt the methods of regulating heart and benefiting brain,replenishing kidney and supplementing essence,and dredging semen chamber,which have yielded definite effects.This article systematically explains the molecular mechanisms of the three methods above in the treatment of BPH,laying a foundation for revealing the biological basis of treating BPH via the“brain-heart-kidney-semen chamber”axis.
