摘要
目的研究中等强度他汀类药物调脂治疗效果对男性冠心病患者肠道菌群的影响。方法选取2019年1月至2022年1月于九江市第一人民医院接受中等强度他汀类药物治疗的60例男性冠心病患者作为研究对象,根据调脂效果分为达标组[低密度脂蛋白胆固醇(LDL-C)<1.8mmol/L,n=36]与未达标组(LDL-C≥1.8mmol/L,n=24)。比较两组血糖血脂指标、肠道菌群Alpha多样性、肠道益生菌数量、肠道致病菌数量及不良反应发生情况。结果治疗后,达标组甘油三脂(TG)、总胆固醇(TC)、LDL-C水平均低于未达标组,差异有统计学意义(P<0.05),但两组高密度脂蛋白胆固醇、空腹血糖、糖化血红蛋白A1c水平比较差异无统计学意义。治疗后,达标组肠道菌群操作分类单元(OTU)、Spannon指数高于未达标组,肠道菌群Simpson指数低于未达标组,差异有统计学意义(P<0.05)。治疗后,达标组肠道益生菌菌群(双歧杆菌、乳酸菌、拟杆菌)数量多于未达标组,肠道致病菌菌群(大肠杆菌、梭菌、链球菌)数量少于未达标组,差异有统计学意义(P<0.05)。治疗后,达标组不良反应发生率低于未达标组,差异有统计学意义(P<0.05)。结论中等强度他汀类药物调脂治疗效果达标,能增加肠道菌群多样性及益生菌数量,减少致病菌数量,降低胃肠道不良反应发生率。
Objective To study the influence of lipid-lowering treatment effect of moderate-intensity statins on intestinal flora in male patients with coronary heart disease(CHD).Methods A total of 60 male patients with CHD who received moderate-intensity statins in Jiujiang No.1 People's Hospital from January 2019 to January 2022 were selected as the research subjects,they were divided into the standard group[low-density lipopro-tein cholesterol(LDL-C)<1.8 mmol/L,n=36]and the non-standard group(LDL-C≥1.8 mmol/L,n=24)according to the lipid-lowering effect.The blood glucose and lipid indicators,alpha diversity of gut microbiota,number of gut probiotics,number of gut pathogenic bacteria,and incidence of adverse reactions were compared between the two groups.Results After treatment,the levels of triglyceride(TG),total cholesterol(TC)and LDL-C in the standard group were lower than those in the non-standard group,and the differences were statistically significant(P<0.05),but there were no significant differences in the levels of high-density lipoprotein cholesterol,fasting blood glucose and glycosylated hemoglobin between the two groups.After treatment,the operational taxonomic unit(OTU)and Spannon index of intestinal flora in the standard group were higher than those in the non-standard group,the Simpson index of intestinal flora was lower than that in the non-standard group,and the differences were statistically sig-nificant(P<0.05).After treatment,the number of intestinal probiotic flora(bifidobacterium,lactobacillus,bacteroidetes)in the standard group was more than that in the non-standard group,the number of intestinal pathogenic bacteria flora(escherichia coli,clostridium,streptococcus)was less than that in the non-standard group,the differences were statistically significant(P<0.05).After treatment,the total incidence rate of adverse reac-tions in the standard group was lower than that in the non-standard group,and the difference was statistically significant(P<0.05).Conclusion Lip-id-lowering treatment effect of moderate-intensity statins is up to standard,which can increase the diversity of intestinal flora and the number of pro-biotics,reduce the number of pathogenic bacteria,and reduce the incidence rates of gastrointestinal adverse reactions.
