摘要
目的 总结分析蓝色橡皮疱痣综合征(BRBNS)患儿的临床特征,并对内镜治疗BRBNS进行系统综述。方法 回顾性收集并分析2010年1月至2023年6月首都医科大学附属北京世纪坛医院和首都医科大学附属北京友谊医院共收治的5例BRBNS患儿的临床特征,包括临床表现、实验室及影像学检查、内镜、治疗和预后等资料,同时系统综述分析既往文献报道的内镜治疗BRBNS文献。结果 5例BRBNS患儿均以黑便为主诉,均有不同程度贫血,病变内镜下特征为直径0.2~1.7 cm大小的蓝紫色、隆起性病变,分布于胃、十二指肠、小肠及结直肠。治疗上,3例行内镜下注射聚桂醇硬化剂治疗有效;1例行内镜+腹腔镜联合治疗有效;1例内镜治疗后仍有出血,行剖腹探查+缝扎治疗后好转。系统综述共纳入病例53例,包括男性27例,女性26例,内镜治疗时采取1种或多种治疗方式,其中34例患者症状逐渐改善,2例经内镜治疗后仍有便血,1例行氩离子凝固术治疗后出现穿孔,转行外科手术后好转。结论 BRBNS是一种罕见的先天性静脉畸形,多以消化道出血起病,内镜下治疗是安全且有效的治疗方法。
Objective To summarize and analyze the clinical features of blue rubber bullous syndrome(BRBNS)in children,and to systematically review the endoscopic treatment of BRBNS.Methods Clinical characteristics of 5 children with BRBNS admitted to Beijing Shijitan Hospital,Capital Medical University and Beijing Friendship Hospital,Capital Medical University from January 2010 to June 2023 were retrospectively collected and analyzed,including clinical manifestations,laboratory and imaging examinations,endoscopy,treatment,and prognosis.The previous reports on endoscopic treatment of BRBNS was systematically reviewed and analyzed at the same time.Results Five children with BRBNS all complained of melena and had varying degrees of anemia.The endoscopic features of the lesions were blue-purple,elevated lesions with a diameter of 0.2-1.7 cm.Endoscopic injection of lauromacrogol sclerotherapy was effective in 3 cases.Endoscopic and laparoscopic treatment was effective in 1 case.One case still had bleeding after endoscopic treatment and was improved after laparotomy and suture ligation.A total of 53 cases were included in the systematic review,including 27 males and 26 females.One or more treatment methods were used during endoscopic treatment,of which 34 cases symptoms were gradually improved,2 cases still had blood in the stool after endoscopic treatment,and 1 case had perforation after argon plasma coagulation treatment,which was improved after surgery.Conclusion BRBNS is a rare congenital venous malformation.The most common symptom is gastrointestinal bleeding.Endoscopic treatment is safe and effective for BRBNS.
