摘要
目的研究小陷胸汤加减治疗肥胖2型糖尿病(T2DM)的临床疗效及对胰岛素抵抗的影响。方法以2022年5月-2023年5月铅山县中医院收治的65例肥胖T2DM患者为研究对象,经随机数字表法分为对照组(32例)与观察组(33例)。对照组给予常规西药治疗,观察组在对照组基础上联合小陷胸汤加减治疗,比较两组临床疗效、血糖指标[空腹血糖(FBG)、餐后2 h血糖(PBG)、糖化血红蛋白(HbA1c)]、中医症状积分、胰岛功能[空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)]、身体质量指数(BMI)与腰臀比。结果观察组治疗总有效率为96.97%,高于对照组的84.38%(P<0.05);两组治疗后FBG、PBG、HbA1c低于治疗前,且观察组FBG、PBG、HbA1c低于对照组(P<0.05);两组治疗后中医症状积分(口渴多饮、易饥多食、脘腹胀满)低于治疗前,且观察组中医症状积分(口渴多饮、易饥多食、脘腹胀满)低于对照组(P<0.05);两组治疗后FINS高于治疗前,HOMA-IR低于治疗前,且观察组FINS高于对照组,HOMA-IR低于对照组(P<0.05);两组治疗后BMI、腰臀比小于治疗前,且观察组BMI、腰臀比小于对照组(P<0.05)。结论小陷胸汤加减治疗肥胖T2DM的疗效确切,可降低患者血糖水平,改善其中医症状与胰岛功能,有利于患者BMI与腰臀比的下降。
Objective To study the clinical efficacy of modified Xiaoxianxiong decoction in the treatment of obese type 2 diabetes mellitus(T2DM)and its effect on insulin resistance.Methods A total of 65 obese T2DM patients admitted to Yanshan County Hospital of Traditional Chinese Medicine from May 2022 to May 2023 were divided into control group(32 patients)and observation group(33 patients)by random number table method.The control group was treated with conventional western medicine,and the observation group was treated with modified Xiaoxianxiong decoction on the basis of the control group.The clinical efficacy,blood glucose index[fasting blood glucose(FBG),postprandial 2 h blood glucose(PBG),glycosylated hemoglobin(HbA1c)],TCM symptom score,islet function[fasting insulin(FINS),insulin resistance index(HOMA-IR)],body mass index(BMI)and waist-to-hip ratio were compared between the two groups.Results The total effective rate of treatment in the observation group was 96.97%,which was higher than 84.38%in the control group(P<0.05).After treatment,FBG,PBG and HbA1c in the two groups were lower than those before treatment,and FBG,PBG and HbA1c in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of TCM symptoms(thirst and polydipsia,hunger and polyphagia,abdominal distension and fullness)in the two groups were lower than those before treatment,and the scores of TCM symptoms(thirst and polydipsia,hunger and polyphagia,abdominal distension and fullness)in the observation group were lower than those in the control group(P<0.05).After treatment,FINS in the two groups was higher than that before treatment,HOMA-IR was lower than that before treatment,and FINS in the observation group was higher than that in the control group,HOMA-IR was lower than that in the control group(P<0.05).The BMI and waist-to-hip ratio of the two groups after treatment were smaller than those before treatment,and the BMI and waist-hip ratio of the observation group were smaller than those of the control group(P<0.05).Conclusion Modified Xiaoxianxiong decoction is effective in the treatment of obese T2DM,which can reduce the blood glucose level,improve their TCM symptoms and islet function,and is conducive to the decline of BMI and waist-to-hip ratio.
