摘要
瘿病发病机制复杂,西医范畴类似于多种甲状腺疾病,如进一步发展易引起严重并发症,中医药在综合长期干预、延缓病情进展方面有明显优势。全国名老中医刘惠民先生从医近六十年,在治疗瘿病方面有独到见解,擅长治疗西药疗效不佳、长久不愈的瘿病。刘老认为单纯性甲状腺肿、甲状腺功能亢进等甲状腺疾病临床可从瘿病分型论治,病多属本虚标实,当责之于肝脾肾,涉及心肺,基本病机为脾肾不足、气血不畅、痰邪阻滞,在临证时受其师张锡纯思想影响,强调固本,注重整体辨证,善于攻补兼施。用药上极具个人特色,重视中药性味、炮制方法、剂型等的选择,常汤剂酒剂并用,并在运用酸枣仁、黄药子等药治疗瘿病方面有丰富经验,文章对刘老治疗瘿病的临证经验进行系统梳理,以期为临床治疗该病提供一定借鉴意义。
The pathogenesis of Ying disease is complex,and the scope of modern medicine is similar to a variety of thyroid diseases,which can easily cause serious complications if further developed.LIU Huimin,a famous veteran Chinese medicine doctor in China,has been practicing medicine for nearly 60 years,and has unique insights in the treatment of Ying disease,and is good at treating Ying disease with poor efficacy of Western medicine and long-term incurability.Mr.LIU believes that simple goiter,hyperthyroidism and other thyroid diseases can be clinically treated from the classification of Ying disease,most of the diseases belongs to deficiency in origin and excess in superficiality,the responsibility should be on the liver,spleen and kidney,involving the heart and lungs,the basic pathogenesis is spleen and kidney insufficiency,phlegm and evil blockage,in the clinical syndrome by his teacher ZHANG Xichun thought influence,emphasizing the emphasize the solid root,pay attention to the overall syndrome differentiation,good at attacking and supplementing.The use of medicine is very personal,attaches importance to the choice of traditional Chinese medicine taste,processing method,dosage form,etc.He often uses decoction and wine together,and has rich experience in the Suanzaoren(Ziziphi Spinosae Semen),Huangyaozi(Airpotato Yam Rhizome)and other drugs in the treatment of Ying disease,this article systematically combs the clinical experience of Mr.LIU in the treatment of Ying disease,in order to provide certain reference significance for the clinical treatment of this disease.
