摘要
目的:基于健康赋权理论护理干预联合多模式镇痛应用于老年股骨颈骨折患者护理中的效果研究.方法:选取2020年1月-2022年12月我院收治72例的老年股骨颈骨折患者为研究对象.按照随机数字表法将其分为观察组和对照组,各36例.对照组采用常规护理,观察组在对照组基础上增加基于健康赋权理论护理干预联合多模式镇痛护理.对比2组在干预前和干预后3个月后的效果差异.结果:干预3个月后,观察组一般自我效能、健康促进生活方式-Ⅱ评分均高于对照组,组间差异有统计学意义(P<0.05).干预2周后,2组皮质醇、去甲肾上腺素、肾上腺素均较干预前降低,且观察组低于对照组,差异有统计学意义(P<0.05).观察组疼痛评分低于对照组,功能、活动评分高于对照组,并发症发生率低于对照组,组间差异有统计学意义(P<0.05).结论:基于健康赋权理论护理干预联合多模式镇痛对老年股骨颈骨折患者效果显著,可提升患者自我效能,降低疼痛带来的应激反应,降低术后并发症发生率,促进远期髋关节功能恢复,使其生活方式更健康.
Objective:To study the effect of nursing intervention combined with multi-mode analgesia based on health empowerment theory in nursing care of elderly patients with femoral neck fracture.Methods:72 elderly patients with femoral neck fracture admitted to our hospital from January 2020 to December 2022 were selected as the study objects.According to random number table method,they were divided into an observation group and a control group,36 cases each.The control group received routine nursing,and the observation group was supplemented with nursing intervention based on health empowerment theory combined with multi-mode analgesic nursing.The effects of the two groups were compared before intervention and 3 months after intervention.Results:3 months after intervention,the General Self-efficacy Health Promotion Lifestyle II scores of the observation group were higher than those of the control group,and cortisol,norepinephrine,adrenalinewere statistically significant(P<0.05).2 weeks after intervention,cortisol,norepinephrine,adrenaline in 2 groups were lower than before intervention,and observation group was lower than control group,the differences were statistically significant between the groups(P<0.05).The pain score of the observation group was lower than that of the control group,the function and activity scores were higher than that of the control group,and the complication rate was lower than that of the control group,with statistical significance between the groups(P<0.05).Conclusion:Nursing intervention combined with multi-mode analgesia based on health empowerment theory has a significant effect on elderly patients with femoral neck fracture,which can improve patients'self-efficacy,reduce the stress response caused by pain,reduce the incidence of postoperative complications,promote long-term hip function recovery,and make their lifestyle healthier.
