摘要
目的:探讨半髋关节与全髋关节置换术治疗创伤性股骨颈骨折患者的临床效果.方法:回顾性分析2021年1月-2022年12月我院收治的60例创伤性股骨颈骨折患者的临床资料,按照随机数字表法将其分为半髋关节置换术组与全髋关节置换术组,各30例.比较2组患者的手术指标及关节恢复效果.结果:半髋关节置换术组手术时间为(90.06±8.46)分钟、住院时间为(14.79±1.46)天,均短于全髋关节置换术组的(131.65±7.01)分钟、(15.22±1.86)天,术中出血量为(193.06±20.42)mL,少于全髋关节置换术组的(276.32±34.20)mL,组间差异有统计学意义(P<0.05).术后5、10个月,全髋关节置换术组的屈髋活动度、外展活动度、外旋活动度均大于半髋关节置换术组,组间差异有统计学意义(P<0.05).结论:半髋关节与全髋关节置换术在治疗创伤性股骨颈骨折患者中具有各自的临床优势,半髋关节置换术组手术指标更优,全髋关节置换术组的治疗效果更佳.因此,在选择治疗方法时根据患者的实际情况选择对应的治疗方案.
Objective:To investigate the clinical effect of semi-hip and total hip arthroplasty in the treatment of traumatic femoral neck fracture.Methods:Clinical data of 60 patients with traumatic femoral neck fracture admitted to our hospital from January 2021 to December 2022 were retrospectively analyzed,and they were divided into a semi-hip arthroplasty group and a total hip arthroplasty group according to random number table method,with 30 cases in each group.The operative indexes and the effect of joint recovery were compared between the two groups.Results:The operative time and hospital stay of the semi-hip arthroplasty group were(90.06±8.46)min and(14.79±1.46)d,which were shorter than(131.65±7.01)min and(15.22±1.86)d of the total hip arthroplasty group,the intraoperative blood loss was(193.06±20.42)mL,it was less than(276.32±34.20)mL in the total hip arthropasty group,the differences between the two groups were statistically significant(P<0.05).5 and 10 months after operation,hip flexion motion,abduction motion and external rotation motion in the total hip arthropasty group were greater than those in the semi-hip arthropasty group,and the differences between the two groups were statistically significant(P<0.05).Conclusion:Semi-hip arthroplasty and total hip arthroplasty have their own clinical advantages in the treatment of traumatic femoral neck fractures.The semi-hip arthroplasty group has better surgical indicators,and the total hip arthroplasty group has better therapeutic effect.Therefore,the corresponding treatment plan is selected according to the actual situation of the patient when selecting the treatment method.
