摘要
目的:探讨老年慢性心力衰竭(CHF)患者应用压力接种训练联合结构式心理干预的效果。方法:本研究纳入某院2021年8月-2023年8月收治的老年CHF患者120例,随机分为两组,对照组(60例)行常规护理,观察组(60例)在其基础上行压力接种训练联合结构式心理干预,对比两组心理水平、希望水平、自我感知压力、生存质量。结果:干预后观察组医院焦虑-抑郁情绪测定表(HAD)、Beck抑郁自评问卷(BDL)得分均低于对照组得分(t=9.053,6.109;P<0.001);干预后观察组积极态度、积极行动及Herth希望评定量表(HHI)总分均高于对照组(t=7.487,9.071,4.112;P<0.001);干预后观察组中文版压力知觉量表(CPSS)失控感、紧张感及总分均低于对照组(t=7.262,8.707,11.349;P<0.001);干预后观察组明尼苏达心力衰竭生活质量量表(MLHFQ)总分低于对照组(t=8.800,P<0.001)。结论:应用压力接种训练联合结构式心理干预能够改善老年CHF患者的心理及自我压力水平,提高希望水平及生存质量,值得临床应用。
Objective:To investigate the effect of stress inoculation training combined with structured psychological intervention on elderly patients with chronic heart failure.Methods:This study included 120 elderly CHF patients admitted to our hospital from August 2021 to August 2023,The control group(60 cases)received routine nursing care,while the observation group(60 cases)received stress vaccination training combined with structured psychological intervention.The psychological level,hope level,perceived stress,and quality of life of the two groups were compared.Results:After intervention,the hospital anxiety depression scale(HAD)and Beck Depression Self Rating Questionnaire(BDL)scores of the observation group were lower than the control group's(t=9.053,6.109;P<0.001);After intervention,the total scores of positive attitude,positive action,and Herth Hope Assessment Scale(HHI)in the observation group were higher than those in the control group(t=7.487,9.071,4.112;P<0.001);After intervention,the Chinese version of the Perceived Stress Scale(CPSS)in the observation group were lower than those in the control group(t=7.262,8.707,11.349;P<0.001);After intervention,the total score of the Minnesota Heart Failure Quality of Life Scale(MLHFQ)in the observation group was lower than that in the control group(t=8.800,P<0.01).Conclusion:Stress inoculation training combined with structured psychological intervention can improve psychological status and self-perceived stress of elderly patients with chronic heart failure,and enhance their hope level and quality of life.
