摘要
目的采用医案数据挖掘的方法对国医大师路志正治疗湿热中阻型胃脘痛病案的临床症状、用药特色、组方规律、药症关系等进行分析。方法收集广安门医院路志正教授特需门诊及三芝堂门诊治疗湿热中阻型胃脘痛病案。采用Epidata 3.1数据管理软件建立医案数据库。采用SPSS Modeler l4.2统计学软件中提供的Apriori算法进行关联规则分析。结果(1)共纳入病历258份,所涉证型6种、治法10种、处方258首,共用药物163味,药物总频数2955次。使用频数15次以上的药物有20味。(2)药物-药物关联规则按支持度≥15%,置信度≥100%,分析出二阶关联规则3条,三阶关联规则13条;症状-药物关联规则按支持度≥10%,置信度≥100%,分析出二阶关联规则9条,三阶关联规则23条,四阶关联规则12条。结论基于数据关联规则挖掘的路志正教授治疗湿热中阻型胃脘痛的药物组合规律,能够反映出路志正教授治疗湿热中阻型胃脘痛运用“宣上、畅中、润下”的学术思想及具体用药特色和治疗方法,为进一步研究路志正教授的学术思想和用药经验提供科学依据和数据参考。
Objective To analyze the clinical symptoms,medication characteristics,compatibility rules,and drug-symptom relationship of the TCM master Lu Zhizheng in treating epigastric pain due to dampness-heat obstructing middle energizer by data mining from medical records.Methods The medical records of the patients with epigastric pain due to dampness-heat obstructing middle energizer treated by Professor Lu Zhizheng in the outpatient department of Guang′anmen Hospital and Sanzhitang Clinic were collected.Epidata 3.1 was used to build a medical case library.The Apriori algorithm in SPSS Modeler 14.2 was used for association rules analysis.Results(1)A total of 258 medical records were included,involving 6 syndromes,10 therapies,258 prescriptions,and 163 medicines with the total frequency of 2955.There were 20 medicines with the frequency above 15.(2)With the thresholds of support≥15%and confidence≥100%,3 second-order association rules and 13 third-order medicine-medicine association rules were mined out.With the thresholds of support≥10%and confidence≥100%,9 second-order association rules,23 third-order association rules,and 12 fourth-order symptom-medicine association rules were mined out.Conclusion The association rules reveal Professor Lu Zhizheng′s academic thought and medication characteristics of ventilating the upper energizer,dredging the middle energizer,and moistening the lower energizer in treating epigastric pain due to dampness-heat obstructing middle energizer.The findings provide a scientific basis and data reference for further research on Professor Lu Zhizheng′s academic thought and medication experience.
