摘要
目的:比较早期非小细胞肺癌(NSCLC)胸腔镜下肺亚段切除术与肺段切除术的治疗效果。方法:选取NSCLC患者160例,根据手术类型不同将患者为肺亚段组(肺亚段切除术)与肺段组(肺段切除术),经倾向性匹配排除混杂因素后,两组各78例。比较两组患者围手术期指标、术后并发症发生情况、术后1年肺功能及3年内无进展生存期(PFS)、总生存期(OS)。结果:与肺段组比较,肺亚段组术中出血量、引流量更多,手术时间更长(均P<0.05)。两组术后并发症发生率比较差异无统计学意义(P>0.05)。术后1年,肺亚段组用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))及每分钟最大通气量(MVV)高于肺段组(均P<0.05)。两组患者3年内PFS及OS比较差异无统计学意义(均P>0.05)。结论:肺亚段切除术与肺段切除术均可有效治疗早期NSCLC,前者在改善患者肺功能方面更具优势,而后者手术时间、术中出血量更少,临床可根据患者病情合理选择术式。
Objective:To compare the efficacy of thoracoscopic lung subsegmentectomy and lung segmentectomy in the treatment of early non-small cell lung cancer(NSCLC).Methods:A total of 160 patients with NSCLC were selected and divided into lung subsegmental group(lung subsegmentectomy)and lung segmental group(lung segmentectomy)according to different types of surgery.After bias matching and excluding confounding factors,78 patients were in each group.Perioperative indexes,postoperative complications,pulmonary function one year after surgery,progression-free survival(PFS)and overall survival(OS)within 3 years were compared between the two groups.Results:Compared with lung segment group,lung subsegment group had more intraoperative blood loss and drainage volume,and the operative time was longer(all P<0.05).There was no significant difference in the incidence of postoperative complications(P>0.05).One year after surgery,FVC,FEV_(1) and MVV in lung subsegmental group were higher than those in lung segment group(all P<0.05).There was no significant difference in PFS and OS between the two groups in 3 years(all P<0.05).Conclusion:Both lung subsegmentectomy and lung segmentectomy can effectively treat early NSCLC.The former one has more advantages in improving the pulmonary function of patients,while the latter one has shorter surgical time and less intraoperative blood loss.Clinically,the surgical method can be reasonably selected according to the disease condition.
