摘要
目的探讨基于数据挖掘的张磊国医大师运用柴胡类方的经验,为经方的临床应用提供参考。方法收集和整理《张磊临证心得集》《张磊医学全书》《国医大师张磊医学文库》及2018年1月至2022年8月国医大师张磊名医工作室运用《伤寒论》柴胡类方的医案,采用WPS Excel 2023及中医传承辅助平台(V2.5)软件,对录入信息进行中医疾病、柴胡类方合方、加味药物等多角度分析。结果纳入医案381个,病例500例,涉及脾胃病证、气血津液病证及肝胆系病证等多种疾病。柴胡类方常搭配的处方有桂枝汤、金铃子散和越鞠丸等,常用药物药性以温、寒为主,药味以辛、甘、苦为主,药物主要归脾、胃、肺、肝、胆经。加味药物以清热药、活血化瘀药为首,加味药物频次排名前5位的有薏苡仁、连翘、金银花、川芎、延胡索,基于复杂系统熵聚类得到18个核心药物组合。结论张磊国医大师将柴胡类方主用于少阳病,根据少阳病易邪结胁下、易相火不足、易气血瘀滞、易气郁化火、易累及太阴、易生痰生饮和易并发太阳阳明的特点,辨证加减用药,常配合使用的方剂有桂枝汤、金铃子散、越鞠丸和橘皮竹茹汤等,常配合使用的药物有薏苡仁、金银花、连翘、川芎、延胡索、郁金、陈皮、川楝子和白术等。
Objective To explore experience of Zhang Lei uses Chaihu classic prescriptions as Chinese medicine master based on data mining,and provide reference for clinical application of classic prescriptions.Methods Medical cases of Zhang Lei’s Collection of Clinical Experience,Zhang Lei’s Complete Book of Medicine,Chinese Medical Master Zhang Lei Medical Library,and Chinese medical master Zhang Lei’s Famous Medical Studio uses Chaihu classic prescriptions of Shanghan Lun from January 2018 to August 2022 were collected and organized.Input informations from multiple perspectives such as traditional Chinese medicine diseases,combined prescriptions of Chaihu classic and flavored drugs were analyzed by using WPS Excel 2023 and Traditional Chinese Medicine Inheritance Assistance Platform(V2.5)software.Results A total of 381 medical records and 500 cases were included,involving various diseases such as spleen and stomach disease syndrome,qi,blood and body fluid disease syndrome and liver and gallbladder disease syndrome.Prescriptions commonly paired with Chaihu classic prescriptions included Guizhi Decoction,Jinlingzi San,Yueju Wan,and so on.The commonly used medicinal properties were mainly warm and cold,and medicinal taste was mainly pungent,sweet,and bitter.Drugs mainly targeted spleen,stomach,lungs,liver,and gallbladder meridians.Flavored drugs mainly focused on clearing heat,promoting blood circulation and removing blood stasis.Top five most frequently used flavored drugs included Coicis Semen,Forsythiae Fructus,Lonicerae Japonicae Flos,Chuanxiong Rhizoma,and Corydalis Rhizoma.Eighteen core drug combinations obtained based on complex system entropy clustering.Conclusion Zhang Lei as master of traditional Chinese medicine mainly uses Chaihu classic prescriptions for Shaoyang disease.Zhang Lei increases or decreases medication based on characteristics of Shaoyang disease,such as easy to be threatened by pathogenic factors,easy to be lack of ministerial fire,easy to be qi and blood stasis,easy to be qi stagnation and fire,easy to involve taiyin,easy to produce phlegm and drink and easy to be complicated with taiyang yangming.Commonly used formulas include Guizhi Decoction,Jinlingzi San,Yueju Wan,and Jupi Zhuru Decoction.Commonly used medications include Coicis Semen,Lonicerae Japonicae Flos,Forsythiae Fructus,Chuanxiong Rhizoma,Corydalis Rhizoma,Curcumae Radix,Citri Reticulatae Pericarpium,Toosendan Fructus,and Atractylodis Macrocephalae Rhizoma,etc.
