摘要
目的探讨血清白细胞介素-17(IL-17)、白细胞介素-8(IL-8)及补体C3、C4水平与系统性红斑狼疮(SLE)活动性的关系。方法选取2018年1月至2023年6月于安徽医科大学第二附属医院就医的72例SLE患者作为试验组,根据SLE疾病活动指数(SLEDAI)将≥5分者作为活动期组(43例),<5分者作为非活动期组(29例),另选取同期体检的60例健康者作为对照组,对比不同组别血清IL-17、IL-8及补体C3、C4水平,通过受试者工作曲线(ROC)评估血清指标在SLE疾病活动性评估中的应用价值。结果试验组血清IL-17、IL-8水平高于对照组,补体C3、C4水平低于对照组(P<0.05);SLE患者活动期血清IL-17、IL-8水平高于非活动期,补体C3、C4水平低于非活动期(P<0.05);血清IL-17、IL-8及补体C3、C4水平联合检测评估SLE疾病活动度的ROC曲线下面积(0.848)高于各项指标单独检测的曲线下面积(0.708、0.705、0.726、0.665)(P<0.05);Pearson相关性分析结果显示,SLE患者SLEDAI积分与血清IL-17、IL-8水平呈正相关,与补体C3、C4水平呈负相关(r=0.526、0.414、-0.503、-0.297,P<0.05)。结论SLE患者血清IL-17、IL-8呈高表达,补体C3、C4水平呈低表达,且其表达水平与患者疾病活动度相关,可在一定程度上评估患者疾病状态。
Objective To explore the relationship between serum interleukin-17(IL-17),interleukin-8(IL-8),complements C3 and C4 levels and the activity of systemic lupus erythematosus(SLE).Methods A total of 72 patients with SLE in the Second Hospital of Anhui Medical University from January 2018 to June 2023 were enrolled in the experimental group.According to their scores of Systemic Lupus Erythematosus Disease Activity Index(SLEDAI),they were further divided into active stage group(n=43,≥5 points)and inactive stage group(n=29,<5 points).And 60 healthy controls were enrolled in the control group.The levels of serum IL-17,IL-8,and complements C3 and C4 were compared among different groups.The application value of the above serum indexes in the diagnosis and activity assessment of SLE was evaluated by ROC curves.Results The levels of serum IL-17 and IL-8 in the experimental group were higher than those in the control group,while the levels of C3 and C4 in the experimental group were lower than those in the control group(P<0.05).The levels of IL-17 and IL-18 in the active stage group were higher than those in the inactive stage group,while the levels of C3 and C4 in the active stage group were lower than those in the inactive stage group(P<0.05).The AUC of the combined detection of IL-17,IL-8,C3 and C4 levels for the evaluation of SLE activity was 0.848,which was higher than that of their single detection(0.708,0.705,0.726,and 0.665,respectively;P<0.05).The results of Pearson correlation analysis showed that the SLEDAI score was positively correlated with the levels of serum IL-17 and IL-18,while negatively correlated with the levels of complements C3 and C4(r=0.526,0.414,-0.503,-0.297,respectively;P<0.05).Conclusion The expression levels of serum IL-17 and IL-8 are up-regulated,while the expressions levels of complements C3 and C4 are down-regulated in SLE patients.The expression levels of the above four indexes are correlated with SLE activity,which can be used to evaluate the disease status in SLE patients to a certain extent.
