摘要
目的:探讨左布比卡因复合右美托咪定蛛网膜下腔阻滞在剖宫产术中的麻醉效果。方法:选择2019年8月—2020年5月于酒泉市人民医院行剖宫产术的产妇160例作为研究对象,以随机数字表法分为两组,各80例。两组均行蛛网膜下腔阻滞,其中对照组采用左布比卡麻醉,观察组采用左布比卡因复合右美托咪定麻醉。比较两组产妇麻醉前(T_(0)),麻醉后1、5、10 min(T_(1)~T_(3)),胎儿娩出后1、5、10 min(T_(4)~T_(6)),手术结束时(T_(7)),平均动脉压(MAP)、心率(HR),血氧饱和度(SpO_(2)),感觉、运动阻滞起效时间,感觉、运动阻滞持续时间,麻醉平面达峰时间,不良反应发生情况。结果:T_(0),两组MAP、HR及SpO_(2)比较,差异无统计学意义(P>0.05);T_(1)~T_(7),两组MAP、HR明显降低,但观察组高于对照组(P<0.05);两组SpO_(2)比较,差异无统计学意义(P>0.05)。观察组感觉、运动阻滞起效时间短于对照组,感觉、运动阻滞持续时间长于对照组,麻醉平面达峰时间早于对照组(P<0.05)。观察组不良反应总发生率低于对照组(P<0.05)。结论:左布比卡因复合右美托咪定蛛网膜下腔阻滞在剖宫产术中的麻醉效果较好,可保持产妇血流动力学平稳,提高麻醉效果,减少不良反应。
Objective:To explore the anesthesia effect of levobupivacaine combined with dexmedetomidine for subarachnoid block during cesarean section.Methods:A total of 160 parturients who underwent cesarean section at Jiuquan People's Hospital from August 2019 to May 2020 were selected as the study subjects.They were divided into two groups according to random number table method,with 80 cases in each group.Subarachnoid block was performed in both groups,with levobupivacaine anesthesia in the control group and levobupivacaine combined with dexmedetomidine in the observation group.The mean arterial pressure(MAP),heart rate(HR)and oxygen saturation(SpO_(2))before anesthesia(T_(0)),at 1 min,5 min and 10 min after anesthesia(T_(1)-T_(3)),at 1 min,5 min and 10 min after fetal delivery(T_(4)-T_(6)),and at the end of operation(T_(7)),as well as sensory and motor block onset time,sensory and motor block duration,anesthesia peak time,and adverse reactions were compared between the two groups.Results:At T_(0),there was no significant difference in MAP,HR and SpO_(2) between the two groups(P>0.05).At T_(1)-T_(7),MAP and HR were significantly decreased in the two groups,but these indexes in the observation group were higher than those in the control group(P<0.05).There was no significant difference in SpO_(2) between the two groups(P>0.05).The onset time of sensory and motor block in the observation group was shorter than that in the control group,the duration of sensory and motor block in the observation group was longer than that in the control group,and the peak time of anesthesia plane in the observation group was earlier than that in the control group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:Levobupivacaine combined with dexmedetomidine for subarachnoid block has a good anesthesia effect during cesarean section,and can maintain maternal hemodynamic stability,improve anesthesia effect and reduce adverse reactions.
