摘要
急性非静脉曲张性上消化道出血(Acute non-variceal upper gastrointestinal bleeding,ANVUGIB)是消化系统最常见的急危重症之一。尽管近年来ANVUGIB的诊断与治疗取得了很大进展,大部分患者经过及时救治后预后良好,但仍存在少数高危患者不能被及时识别,进而延误治疗。最新国内外多个指南都推荐使用评分系统进行风险评估以达到高危识别、分层治疗和改善预后的目的。目前,许多评分系统已在临床实践中得到应用,并在评估患者病情和预后方面展现出良好的预测价值。本文就ANVUGIB现有的主流以及部分新近提出的评分系统进行回顾,旨在为消化科及急诊科医师在ANVUGIB患者的诊疗过程中提供新的思路。
Acute non variceal upper gastrointestinal bleeding(ANVUGIB)is one of the most common acute and critical illnesses in the digestive system.Although significant progress has been made in the diagnosis and treatment of ANVUGIB in recent years,with the majority of patients having a good prognosis after timely treatment,there are still a few high-risk patients who cannot be identified in a timely manner,resulting in delayed treatment.Therefore,the latest domestic and international guidelines recommend the use of scoring systems for risk assessment to achieve the goals of high-risk identification,stratified treatment,and improved prognosis.So far,many scoring systems have been applied in clinical practice,showing good predictive value in evaluating patient condition and prognosis.This article reviews the existing mainstream and some newly proposed scoring systems for ANVUGIB,intended to provide new insights for gastroenterologists and emergency physicians in the diagnosis and treatment of ANVUGIB patients.
