摘要
目的探究子痫前期(PE)患者胎盘组织内皮素-1(ET-1)、Notch1蛋白、微小核糖核酸-204(miR-204)表达水平与胎盘螺旋动脉重铸及不良妊娠结局的关系。方法将2021年2月至2023年2月在空军军医大学唐都医院产科建卡并首次确诊为PE的98例患者纳入PE组,另将50例正常妊娠孕妇纳入对照组,比较两组基线资料以及胎盘组织ET-1、Notch1蛋白及miR-204表达水平,比较不同病情程度PE患者胎盘组织ET-1、Notch1蛋白及miR-204表达水平,比较两组胎盘螺旋动脉重铸相关指标(管壁厚度、管腔面积)、妊娠结局(胎盘重量、1 min Apgar评分、新生儿出生体重),分析PE患者胎盘组织ET-1、Notch1、miR-204与胎盘螺旋动脉管壁厚度、管腔面积及妊娠结局的相关性。结果与对照组比较,PE组胎盘组织Notch1蛋白表达水平显著降低,ET-1蛋白及miR-204表达水平显著升高(P<0.05);与PE轻度组比较,PE重度组胎盘组织Notch1蛋白表达水平显著降低,ET-1蛋白及miR-204表达水平显著升高(P<0.05);与对照组比较,PE组管壁厚度显著升高,管腔面积、胎盘重量、1 min Apgar评分、新生儿出生体重显著降低,流产和胎儿生长受限发生率明显升高(P<0.05);Pearson检验显示,PE患者胎盘组织ET-1、miR-204表达水平与管壁厚度、流产和胎儿生长受限发生率呈正相关,与管腔面积、胎盘重量、1 min Apgar评分、新生儿出生体重呈负相关(P<0.05);PE患者胎盘组织Notch1表达水平与管壁厚度、流产和胎儿生长受限发生率呈负相关,与管腔面积、胎盘重量、1 min Apgar评分、新生儿出生体重呈正相关(P<0.05)。结论PE患者胎盘组织ET-1、miR-204呈高表达,Notch1呈低表达,三者表达水平与PE严重程度、胎盘螺旋动脉重铸及孕妇不良妊娠结局相关。
Objective To investigate the relationship between the expression levels of endothelin⁃1(ET⁃1),Notch1 protein,and microribonucleic acid⁃204(miR⁃204)in placental tissue of patients with preeclampsia(PE)and placental spiral artery recasting and adverse pregnancy outcomes.Methods 98 patients who were first diagnosed with PE in Tangdu Hospital,Air Force Military Medical University from February 2021 to February 2023 were included in the PE group,and 50 normal pregnant women were included in the control group.Baseline data and the expression levels of ET⁃1,Notch1 protein,and miR⁃204 in placental tissue were compared between the two groups.The expression levels of ET⁃1,Notch1 protein,and miR⁃204 in placental tissue of PE patients with different degrees of illness were also compared,compared two groups of placental spiral artery recasting related indicators(wall thickness,lumen area)and pregnancy outcomes(placental weight,1⁃minute Apgar score,newborn birth weight)and analyzed the correlation between placental tissue ET⁃1,Notch1,miR⁃204,placental spiral artery wall thickness,lumen area,and pregnancy outcomes in PE patients.Results Compared with the control group,the expression level of Notch1 protein in the placental tissue of the PE group was significantly reduced,while the expression levels of ET⁃1 protein and miR⁃204 were significantly increased(P<0.05).Compared with the mild PE group,the expression level of Notch1 protein in the placental tissue of the severe PE group was significantly reduced,while the expression levels of ET⁃1 protein and miR⁃204 were significantly increased(P<0.05);compared with the control group,the PE group showed a significant increase in tube wall thickness,a significant decrease in lumen area,placental weight,1⁃minute Apgar score,and neonatal birth weight,and a significant increase in the incidence of miscarriage and fetal growth restriction(P<0.05);Pearson's test showed that the expression levels of ET⁃1 and miR⁃204 in placental tissue of PE patients were positively correlated with tube wall thickness,incidence of miscarriage and fetal growth restriction,and negatively correlated with tube area,placental weight,1⁃minute Apgar score,and neonatal birth weight(P<0.05);the expression level of Notch1 in placental tissue of PE patients was negatively correlated with tube wall thickness,incidence of miscarriage and fetal growth restriction,and positively correlated with lumen area,placental weight,1⁃minute Apgar score,and neonatal birth weight(P<0.05).Conclusion ET⁃1 and miR⁃204 are highly expressed in placental tissue of PE patients,while Notch1 is low expressed.The expression levels of these three factors are related to the severity of PE,placental spiral artery recasting,and adverse pregnancy outcomes in pregnant women.
