摘要
目的:检索分析并总结急性失代偿心力衰竭(ADHF)病人Ⅰ期运动康复的最佳证据,为临床制定科学、规范的ADHF病人Ⅰ期运动康复方案提供依据。方法:计算机检索BMJ Best Practice、the Cochrane Library、CINAHL Complete、PubMed、Web of Science、中国知网、万方数据库、维普数据库、中国生物医学文献服务系统等数据库中与ADHF病人运动康复相关的临床指南、专家共识、证据总结、系统评价及Meta分析以及设计良好的随机对照试验等。检索时限为建库至2022年7月5日。提取相关主题及证据,进行循证小组论证。结果:共纳入11篇文献,其中指南4篇、专家共识3篇、随机对照试验2篇、回顾性队列研究2篇,结合循证护理小组论证,汇总形成了30条证据,包括ADHF病人Ⅰ期运动康复的启动时机、评估、运动程序、运动形式、运动强度、运动时间及频率6个方面。结论:总结了ADHF病人Ⅰ期运动康复的最佳证据,为临床心脏康复团队提供了循证依据,建议临床心脏康复团队根据ADHF病人年龄、生命体征、症状体征、心功能改善程度、情绪状态及运动耐受性等进行全面细致的评估后,尽早制定个性化的运动康复方案,提高病人心肺适应性、改善心功能、提高运动能力、缩短住院天数。
Objective:To analyze and summarize the best evidence of phaseⅠexercise rehabilitation in patients with acute decompensated heart failure(ADHF),and to provide a basis for the clinical development of scientific and standardized phaseⅠexercise rehabilitation program for ADHF patients.Methods:BMJ Best Practice,the Cochrane Library,CINAHL Complete,PubMed,Web of Science,CNKI,WanFang Database,VIP and SinoMed were searched for clinical guidelines,expert consensus,evidence summary,systematic evaluation and Meta-analysis related to exercise rehabilitation in patients with ADHF,as well as well-designed randomized controlled trials.The retrieval time was from the inception to July 5,2022.Extract relevant topics and evidence for evidence-based group demonstration.Results:A total of 11 articles were included,including 4 guidelines,3 expert consensus,2 randomized controlled trials and 2 retrospective cohort studies.Combined with the evidence-based nursing group,30 pieces of evidence including 6 aspects of the start time,evaluation,exercise procedure,exercise form,exercise intensity,exercise time and frequency of phaseⅠexercise rehabilitation in ADHF patients were summarized.Conclusion:This study summarizes the best evidence of phaseⅠexercise rehabilitation in ADHF patients,and provides evidence-based evidence for clinical cardiac rehabilitation teams.It is recommended that clinical cardiac rehabilitation teams conduct a comprehensive and detailed assessment based on the age,vital signs,symptoms and signs,cardiac function improvement,emotional state and exercise tolerance of ADHF patients,and formulate personalized exercise rehabilitation programs as soon as possible to improve patients′cardiopulmonary adaptability,improve cardiac function,improve exercise capacity,and shorten hospitalization days.
