摘要
目的 分析不同入路内固定术治疗旋后外旋型Ⅳ度踝关节骨折患者的临床疗效。方法 非随机选取2021年6月—2023年7月济宁市兖州区人民医院收治的70例旋后外旋型Ⅳ度踝关节骨折患者为研究对象,根据手术方式不同分为对照组与研究组,各35例。对照组实施经腓骨入路内固定术治疗,研究组实施经腓骨后外侧入路内固定术治疗。对比两组临床疗效及足功能评分系统评分、并发症发生率。结果 研究组术中出血量少于对照组,手术时间、住院时间、骨折愈合时间均短于对照组,差异有统计学意义(P均<0.05);研究组踝关节优良率高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为5.71%(2/35),低于对照组的22.86%(8/35)差异有统计学意义(χ^(2)=4.200,P<0.05)。结论 在旋后外旋型Ⅳ度踝关节骨折内固定手术治疗中,采用经腓骨后外侧入路临床疗效较优,术中对患者损伤较小,有利于患者术后康复,且能够提高踝关节功能,降低并发症风险。
Objective To analyze the clinical efficacy of different approaches in patients with posterior spiral IV ankle fractures.Methods A total of seventy patients with IV degree ankle fracture of supination external rotation treated in Yanzhou District People′s Hospital of Jining City from June 2021 to July 2023 were non-randomly selected as the study objects,and were divided into control group and study group according to different surgical methods,with thirty-five cases in each group.The control group underwent transfibula approach and the study group underwent transfibula posterolateral approach.The clinical efficacy,the score of foot function scoring system and the incidence of complica-tions were compared between the two groups.Results The intraoperative blood loss of the study group was less than that of the control group,and the operation time,hospital stay and fracture healing time were all shorter than that of the control group,the differences were statistically significant(all P<0.05).The excellent and good rate of ankle joint in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).The complication rate of the study group was 5.71%(2/35),which was significantly lower than 22.86%(8/35)of the control group,the difference was statistically significant(χ^(2)=4.200,P<0.05).Conclusion In the treatment of IV degree ankle fracture with supination external rotation,transfibula posterolateral approach has better clinical efficacy,less intraop-erative injury to patients,which is conducive to postoperative rehabilitation of patients,and can improve ankle joint function and reduce the risk of complications.
