摘要
目的探讨慢性唇炎中医外治的用药规律。方法检索中国知网、万方、维普、中国生物医学、PubMed等数据库中以中药外用治疗慢性唇炎的文献,检索时间为建库至2024年1月,对纳入处方中的中药及其性味归经进行频数分析、聚类分析、关联规则分析。结果根据纳入标准和排除标准对相关文献进行检索和整理,共纳入文献50篇,涉及中药86味,前10味高频药物为苦参、白鲜皮、蛇床子、紫草、当归、地肤子、甘草、黄柏、黄连、冰片;用药以清热药、补虚药、解表药为主,分别占54.49%、14.37%、9.58%;药性分布上以寒为主,占53.33%;药味分布以甘、苦、辛为主,分别占45.45%、27.27%、22.73%。药物归经上以胃、胆、肺、大肠经为主,分别占16.67%、16.67%、14.29%、14.29%。聚类分析大致分为3类。关联规则分析结果显示主要强配伍关系是白鲜皮-苦参。其次中药外治慢性唇炎的剂型,以软膏、水剂占比较高,分别为42.31%、36.54%。结论中药外治慢性唇炎从脾胃湿热、阳明胃经风火为主入手,注重清热、活血化瘀为主,兼顾解表、补虚的治法。药味以选用甘苦为主,药性注重寒、温、平的运用,此为临床中药外治法治疗慢性唇炎提供理论参考。
Objective To explore the medication rule of external treatment of chronic cheilitis with traditional Chinese medicine.Methods Literature on topical treatment of chronic cheilitis with traditional Chinese medicine was searched from CNKI,Wanfang,VIP,China Biomedicine,PubMed and other databases,and the search time was from the establishment of the database to January 2024.Further frequency analysis,cluster analysis,and association rule analysis were performed on the traditional Chinese medicine and its properties,flavors,and meridians in the retrieved prescriptions.Results Relevant articles were searched and organized according to the inclusion and exclusion criteria,with a total of 50 articles included,involving 86 traditional Chinese medicines.The top-10 high-frequency drugs were Sophora flavescens,Dictamnus dasycarpus,Fructus cnidii,Radix lithospermi,Angelica sinensis,Kochia scoparia,Licorice,Phellodendron amurense,Coptis chinensis,and Borneol.The medication was dominated by heat-clearing drugs,deficiency-tonifying drugs,and drugs for relieving exterior syndrome,accounting for 54.49%,14.37%,and 9.58%,respectively;the medicinal property was mainly cold,accounting for 53.33%;and the medicinal flavors were mainly sweet,bitter,and pungent,which were 45.45%,27.27%,and 22.73%,respectively.Furthermore,medicines primarily acted on the stomach,gallbladder,lung,and large intestine meridians,which were 16.67%,16.67%,14.29%and 14.29%,respectively.Three categories could be roughly established based on the cluster analysis.Association rule analysis revealed a major strong compatibility relationship between Dictamnus dasycarpus and Sophora flavescens.In addition,the dosage forms of traditional Chinese medicine for external treatment of chronic cheilitis were generally ointments and water-based agents,accounting for 42.31%and 36.54%,respectively.Conclusion The external treatment of chronic cheilitis with traditional Chinese medicine emphasizes mainly on damp heat in the spleen and stomach,and pathogenic factors of wind and fire of Yangming stomach meridian,with special attention paid to clearing heat,promoting blood circulation and removing blood stasis,combined with therapeutic methods of relieving exterior syndrome and tonifying deficiency.The commonly used medicines are sweet and bitter in taste,as well as cold,warm and neutral in property.Findings in this study provide potential theoretical reference for the clinical application of traditional Chinese medicine for external treatment of chronic cheilitis.
