摘要
目的分析人工真皮修复治疗手指末节指腹皮肤软组织缺损对创面愈合、指腹感觉及血液流变学指标的影响。方法选取2021年1月—2023年12月于郑州市骨科医院治疗的78例手指末节指腹皮肤软组织缺损患者临床资料。随机数字表法分组。对照组采用游离植皮治疗。观察组采用人工真皮修复治疗,对比两组患者临床治疗效果。结果观察组移植物存活率、手术次数和住院时间与对照组比较差异无统计学意义(P>0.05)。对照组手术时间、创面愈合时间长于观察组,且手术费用低于观察组(P<0.05)。观察组指腹感觉等级达到S3+与对照组差异无统计学意义(P>0.05),指腹两点辨别觉及TAM评分与对照组差异无统计学意义(P>0.05)。观察组治疗后红细胞压积、全血黏度显著低于对照组,差异有统计学意义(P<0.05)。结论人工植皮手术相对于传统方法对全血黏度、红细胞压积有更小的影响,可为促进创面愈合创造良好的条件,同时指腹感觉相关功能的恢复与游离植皮修复术效果相当。
Objective To analyze the effects of artificial dermal repair on wound healing,finger pulp sensation and hemorheological indexes in the treatment of finger pulp soft tissue defects of distal fingers.Methods Clinical data of 78 patients with finger pulp soft tissue defects of distal fingers treated in Zhengzhou Orthopedic Hospital between January 2021 and December 2023 were selected with random number table method for grouping.The control group were treated with free skin grafting,while the observation group were treated with artificial dermal repair.The clinical treatment effect of the two groups were compared.Results There was no significant difference in graft survival rate,number of surgeries and length of hospital stay between observation group and control group(P>0.05).The operation time and wound healing time in the control group were longer than those in the observation group,and the operation cost was lower than that in the observation group(P<0.05).There was no significant difference between the observation group and the control group in the finger pulp sensation level reached S3+(P>0.05),and there was no significant difference between the observation group and the control group in the finger pulp two-point discrimination and TAM score(P>0.05).After treatment,whole blood viscosity and hematocrit in the observation group were significantly lower than those of the control group,and the difference was significant(P<0.05).Conclusion Compared with traditional methods,artificial skin graft surgery has less influence on whole blood viscosity and hematocrit,which can create good conditions for promoting wound healing.Meanwhile,the recovery of finger pulp feeling related function is equivalent to that of free skin graft repair.
