摘要
目的探究宫颈癌患者血清miR-182和miR-21表达与人乳头瘤病毒(HPV)感染的相关性。方法选择2020年4月至2023年4月行宫颈活检的120例患者为研究对象,将确诊宫颈癌患者作为宫颈癌组(n=49),确诊宫颈上皮内瘤变(CIN)患者作为CIN组(n=39),确诊宫颈炎患者作为宫颈炎组(n=32)。采用实时定量反转录聚合酶链反应检测血清miR-182、miR-21水平;采用聚合酶链反应扩增技术和第二代杂交捕获法检测HPV-DNA和HPV E6/E7 mRNA载量;采用多因素Logistic回归分析影响宫颈癌发生的因素;采用Spearman法分析宫颈癌患者血清miR-182和miR-21水平与HPV感染的相关性。结果宫颈癌组血清miR-182和miR-21水平均高于CIN组和宫颈炎组,CIN组血清miR-182和miR-21水平均高于宫颈炎组(P<0.05);宫颈癌组、CIN组和宫颈炎组HPV-DNA阳性表达率和HPV E6/E7 mRNA高载量率依次降低(P<0.05);宫颈癌患者血清miR-182和miR-21水平与HPV-DNA表达和HPV E6/E7 mRNA载量呈正相关(P<0.01);miR-182、miR-21、HPV-DNA表达、HPV E6/E7 mRNA载量均是宫颈癌发生的独立危险因素(P<0.01)。结论宫颈癌患者血清miR-182、miR-21水平升高,与宫颈HPV感染密切相关。
Objective To investigate the correlation between serum miR-182 and miR-21 expression and human papillomavirus(HPV)infection in patients with cervical cancer(CC).Methods In total,120 patients who underwent cervical biopsy from April 2020 to April 2023 were selected as the research subjects.Patients with confirmed CC were selected as CC group(n=49),patients with confirmed cervical intraepithelial neoplasia(CIN)as CIN group(n=39),and patients with confirmed cervicitis as cervicitis group(n=32).The serum levels of miR-182 and miR-21 were detected by real-time quantitative reverse transcription polymerase chain reaction,and HPV-DNA and HPV E6/E7 mRNA load were detected by polymerase chain reaction amplification and second-generation hybridization capture test.Multivariate Logistic regression was used to analyze the factors affecting the occurrence of CC.Spearman method was used to analyze the correlation between serum miR-182 and miR-21 levels and HPV infection in patients with CC.Results The serum miR-182 and miR-21 levels in CC group were higher than those in CIN group and cervicitis group,and the serum miR-182 and miR-21 levels in CIN group were higher than those in cervicitis group(P<0.05).The positive expression rate of HPV-DNA and the high load rate of HPV E6/E7 mRNA in CC group,CIN group and cervicitis group were decreased successively(P<0.05).Serum miR-182 and miR-21 levels were positively correlated with HPV-DNA expression and HPV E6/E7 mRNA load in patients with CC(P<0.01).miR-182,miR-21,HPV-DNA expression,and HPV E6/E7 mRNA load were independent risk factors for CC(P<0.01).Conclusion The elevated levels of miR-182 and miR-21 in patients with CC are closely related to cervical HPV infection.
