摘要
目的探讨聚焦解决模式对首发艾滋病患者心理弹性、急性应激反应及生存质量的影响。方法采用便利抽样法选取2021年9月至2023年8月在广西壮族自治区某三级甲等医院收治的首发艾滋病患者109例为研究对象,分为试验组(n=56)和对照组(n=53)。对照组给予入院宣教、症状护理、微信回访等常规护理,试验组在常规护理的基础上接受聚焦解决模式干预。两组患者采用心理弹性量表(CD-RISC)、斯坦福急性应激反应问卷(SASRQ)、世界卫生组织艾滋病生存质量测量简表(WHOQOL-HIV BREF)进行评定。结果干预后1周、1个月试验组患者CD-RISC总分以及坚韧性维度、力量性维度、乐观性维度评分均较对照组高(P<0.001);干预后1周、1个月试验组患者SASRQ总分以及分离、再体现、回避、激惹、功能损伤评分均较对照组低(P<0.05);干预6个月后,试验组患者在WHOQOL-HIV BREF总分及生理领域、社会关系领域、心理领域、环境领域、独立性领域评分均较对照组高(P<0.001),但在精神支柱领域差异无统计学意义(P=0.466)。结论聚焦解决模式有助于提高首发艾滋病患者心理弹性,缓解其急性应激反应,改善生存质量。
Objective To explore the impact of solution-focused approach on resilience,acute stress response,and quality of life among patients with first-onset HIV/AIDS.Methods A convenience sampling method was used to select 109 patients with first-onset HIV/AIDS(research objects)who admitted to a tertiary hospital in Guangxi Zhuang Autonomous Region from September 2021 to August 2023.They were divided into experimental group(n=56)and control group(n=53).The control group were given routine care including inpatient education,symptom-related caring,and WeChat follow-up,while the experimental group were intervened with the solution-focused approach on the basis of routine care.Both groups were evaluated by the Connor-Davidson Resilience Scale(CD-RISC),Stanford Acute Stress Reaction Questionnaire(SASRQ),and the World Health Organization Quality of Life HIV Brief Scale(WHOQOL-HIV BREF).Results One week and one month after intervention,the total score of CD-RISC,as well as the scores of resilience,strength,and optimism in the experimental group were higher than those in the control group(P<0.001).One week and one month after intervention,the total scores of SASRQ,as well as the scores of separation,re-manifestation,avoidance,irritability,and functional impairment in the experimental group were lower than those in the control group(P<0.05).After six months of intervention,the experimental group had higher total scores of WHOQOL-HIV BREF and scores in physiological,social relationship,psychological,environmental,and independence domains compared to the control group(P<0.001),but there was no statistically significant difference in the domain of mental support(P=0.466).Conclusion Solution-focused approach can help to improve psychological resilience,alleviate acute stress response,and improve the quality of life among patients with first-onset HIV/AIDS.
