摘要
目的比较舒芬太尼联合不同剂量纳布啡用于剖宫产术后镇痛的效果。方法回顾性抽取2021年6月至2023年2月郑州大学第二附属医院收治的96例行剖宫产的产妇为研究对象,术后均应用自控静脉镇痛泵镇痛。按照镇痛泵药物配制的不同将产妇分为A、B、C组三组,每组32例。各组产妇给予舒芬太尼的剂量相同(1.0μg/kg),A、B、C三组纳布啡用药剂量分别为1.0、1.5、2.0 mg/kg。比较三组产妇术后疼痛及镇静情况、总不良反应发生率及镇痛满意度。结果B组与C组术后各个时间点的镇痛及镇静效果均优于A组(P均<0.05);B、C组的总不良反应发生率均低于A组(P均<0.05),而B组总不良反应发生率最低;B组总满意度(93.75%,30/32)和C组总满意度(87.50%,28/32)均高于A组(65.63%,21/32),P均<0.05,且B组高于C组(P>0.05)。结论将1.5 mg/kg纳布啡与1.0μg/kg舒芬太尼联合用于剖宫产术后镇痛,能够获得满意的镇痛及镇静效果,且安全可靠。
Objective To compare the effects of sufentanil combined with different doses of nalbuphine in postoperative analgesia for cesarean section.Methods A total of 96 parturients who underwent cesarean section in the Second Affiliated Hospital of Zhengzhou University from June 2021 to February 2023 were retrospectively selected as the study subjects,and they used self-control intravenous analgesia pump for postoperative analgesia.According to the formulations of analgesic pump drugs,the parturients were divided into three groups:groups A,B,and C,with 32 cases in each group.The dosage of sufentanil was the same in all groups(1.Oμg/kg),and the dosages of nalbuphine in groups A,B and C were 1.0 mg/kg,1.5 mg/kg and 2.0 mg/kg,respectively.The postoperative pain and sedation,overall incidence of adverse reactions,and satisfaction with analgesia were compared among the three groups.Results The analgesic and sedative effects at each postoperative time point of groups B and C were better than those of group A(all P<0.05).The total incidences of adverse reaction of groups B and C were lower than that of group A(all P<0.05),while the total incidence of adverse reaction of group B was the lowest.The total satisfaction of group B(93.75%,30/32)and group C(87.50%,28/32)were higher than that of group A(65.63%,21/32),all P<0.05;and total satisfaction of group B was higher than that of group C(P>0.05).Conclusions Using 1.5 mg/kg nalbuphine combined with 1.0μg/kg sufentanil in postoperative analgesia for cesarean section can achieve ideal analgesic and sedative effects,moreover,it is safe and reliable.
