摘要
目的分析跨理论模型联合个案管理模式在慢性肾脏病(CKD)患者护理康复管理中的应用效果。方法选择在我院CKD管理中心的300例CKD患者为研究对象,将其随机分为常规组和研究组,各150例。常规组接受常规护理,研究组在常规护理的基础上实施跨理论模型联合个案管理模式。比较两组的干预效果。结果干预后,研究组的自我管理行为能力评分、心理状况评分、肾功能指标、生活质量评分优于常规组(P<0.05)。研究组的纽卡斯尔护理满意度量表(NSNS)评分高于常规组(P<0.05)。结论跨理论模型联合个案管理模式能够提高CKD患者自我管理行为能力与满意度,缓解不良心理状态,改善肾功能与生活质量。
Objective To analyze the application effect of cross-theoretical model combined with case management model in nursing rehabilitation management of patients with chronic kidney disease(CKD).Methods A total of 300 CKD patients in CKD management center of our hospital were selected as the research objects,and the patients were randomly divided into routine group and study group,with 150 cases in each group.The routine group received routine nursing,and the study group implemented cross-theoretical model combined with case management model on the basis of routine nursing.The intervention effects of the two groups were compared.Results After intervention,the self-management behavior ability score,psychological status scores,renal function indexes and quality of life score of the study group were better than those of the routine group(P<0.05).The Newcastle Satisfaction with Nursing Scale(NSNS)score of the study group was higher than that of the routine group(P<0.05).Conclusion The cross-theoretical model combined with case management model can improve the self-management behavior ability and satisfaction of CKD patients,alleviate adverse psychological state,and improve renal function and quality of life.
