摘要
目的探析超声联合CT检查在结节性甲状腺肿合并甲状腺癌患者诊断中的诊断价值。方法选取2022年1月—2023年12月高唐县中医院收治的80例疑似甲状腺癌患者为研究对象,所有入组患者均行超声检查、CT检查,以病理检查结果作为金标准,对比两种检验方式的单一及联合诊断效能与诊断符合率。结果联合诊断的灵敏度为91.80%(56/61)、特异度为94.74%(18/19)、准确度为92.50%(74/80)、阳性预测值为98.25%(56/57)、阴性预测值为78.26%(18/23),均高于超声检查的68.85%(42/61)、63.16%(12/19)、67.50%(54/80)、85.71%(42/49)、38.71%(12/31)与CT检查的77.05%(47/61)、57.89%(11/19)、72.50%(58/80)、85.45%(47/55)、44.00%(11/25),差异有统计学意义(χ^(2)=17.487、7.473、16.057、6.605、9.188,P均<0.05)。联合诊断对乳腺状癌、滤泡癌的的诊断符合率高于单一超声及CT检查,差异有统计学意义(P均<0.05)。不同检查方式对滤泡癌、髓样癌、未分化癌的诊断符合率比较,差异无统计学意义(P均>0.05)。结论超声与CT联合诊断结节性甲状腺肿合并甲状腺癌的诊断效能较高,均可为后续诊治提供一定参考依据。
Objective To explore the diagnostic value of ultrasound combined with CT in the diagnosis of nodular goiter complicated with thyroid cancer.Methods A total of 80 patients with suspected thyroid cancer admitted to Gaotang County Traditional Chinese Medicine Hospital from January 2022 to December 2023 were selected as the research objects.All the enrolled patients underwent ultrasound examination and CT examination.The pathological examination results were used as the gold standard.The single and combined diagnostic efficacy and diagnostic coincidence rate of the two test methods were compared.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined diagnosis were 91.80%(56/61),94.74%(18/19),92.50%(74/80),98.25%(56/57)and 78.26%(18/23),respectively,which were higher than those of ultrasonography[68.85%(42/61),63.16%(12/19),67.50%(54/80),85.71%(42/49)and 38.71%(12/31)]and the CT examination[77.05%(47/61),57.89%(11/19),72.50%(58/80),85.45%(47/55),44.00%(11/25)],and the differences were statistically significant(χ^(2)=17.487,7.473,16.057,6.605,9.188,all P<0.05).The diagnostic coincidence rates of combined diagnosis of breast cancer was higher than that of single ultrasound and CT examination,and the differences was statistically significant(both P<0.05).There was no significant difference in the diagnostic coincidence rate of follicular carcinoma medullary carcinoma and undifferentiated carcinoma between different examination methods(all P>0.05).Conclusion The diagnostic efficacy of ultrasound combined with CT in the diagnosis of nodular goiter complicated with thyroid cancer is high,which can provide a reference for subsequent diagnosis and treatment.
